Central Community Health Centre, 15 Years by Peri Hayes Since 2022, Diane Arsenijevic has been the Program Coordinator at the Central Community Health Centre (CCHC). Having worked at West Elgin Community Health Centre for over 25 years and her beliefs aligning with CCHC, Diane was thrilled to transition to this local organization: “To become part of the program development here was something of high interest.” Behaving as a primary care provider to many clients, CCHC is a vital resource in the community. While striving to improve the health and well-being of individuals who face barriers accessing services, client examples include those without health insurance or with low income, as well as those who are new to the community, experiencing cultural, geographic or language barriers and more. “CCHC helps to provide the needed services and programs for individuals to lead healthier and higher quality lives.” Diane proceeds to highlight the implementation of their Integrated Model of Care. In addition to utilizing the support of doctors and nurse practitioners, rostered clients can access a dietician, physiotherapist and social workers. Due to the current status of healthcare accessibility, CCHC has directed funding toward supporting patients who do not have a primary care provider. “Working alongside the Thames Valley Health Team with funding received to establish the Elgin Community Health Hub, a portion of this funding was provided to our Centre to hire a nurse practitioner who visits surrounding communities three times a week in our mobile health unit.” Represented by a bilingual PSW and a nurse practitioner, this unit also travels on a weekly basis to Aylmer and provides
