When you visit our office Your time is best utilized when you show up to your visit well prepared. This includes completing your intake forms and reviewing our office policies in advance of your visit so we can also prepare ourselves to assist you. Please bring in supplements or prescription medications you are taking or have used in the recent past. Please gather any records of recent labwork, surgeries, ER, hospital or doctor’s office visits or other information that may be useful to us. Our office can assist you in requesting records. Keep your appointment as scheduled, if you can not, please cancel early and reschedule.
We are a Family Practice office and provide services in Internal Medicine, Pediatrics, and Gynecology. We treat patients with ADHD, Anxiety, Depression and Opioid dependency. Our office provides medical and physical therapy care for patients with injuries from car accidents. We also offer cosmetic Juvederm injections to smooth away facial wrinkles.
Please always advise us of any changes in your name, address, telephone number and medical insurance. Please keep your insurance carrier information current including name and telephone number and ID number.
