Who We Are and What We Do Community Engagement HUB Program

Community Health Network of Connecticut, Inc. ® (CHNCT), on behalf of the HUSKY Health program, has established a network of Community Engagement HUBs throughout the state. A HUB is a neighborhood space where members can get H elp U nderstanding their B enefits. Through these HUBs, CHNCT has established a trusted presence in communities with the highest HUSKY Health member populations, making it easier to reach members who may be unaware of all the community services and resources available to them. Members who visit the HUBs are empowered to make better-informed decisions about their health after being introduced to resources to help meet their basic needs. They can also learn how to live a healthier lifestyle.

CHNCT has collaborated with community partners such as Connecticut Foodshare and Workforce Alliance to establish HUB sites where community partners are located. Partners can refer their clients, who are largely comprised of HUSKY Health members, to our information tables at the sites and at community partner events. Examples of events include community meetings, health fairs, parent-teacher nights, and mobile food pantries. HUB events are posted on the HUSKY Health member website and social media pages to let members know when and where they can go to take advantage of these resources. HUBs are generally open for one to two hours at a time and are managed by CHNCT’s Community Health Workers (CHWs).

The HUSKY Health program continues to see positive results from establishing HUB sites in community settings that HUSKY Health members frequently visit and trust. Community partners benefit by increasing the number of resources they can offer their clients, which increases community and member engagement. HUSKY Health members benefit by obtaining information that can help them address social determinants of health (SDOH), including food insecurity, housing, education, and employment. Through resource sharing and working together to impact SDOH, the program improves health equity among HUSKY Health populations throughout Connecticut.

Community Health Worker Ambassador Program Our CHWs help people who struggle with basic needs and use of the healthcare system. They listen to individual needs and concerns and can provide members with the best resources to navigate their situation. Many of our CHWs live and work in the same neighborhoods as our members and have diverse cultural and linguistic backgrounds. Our CHWs serve as ambassadors for the HUSKY Health program. They are committed to building equity through diversity, inclusion, and community engagement efforts. CHWs work to help members reach positive health outcomes regardless of ability, age, cultural background, ethnicity, faith, gender, gender identity, ideology, income, national origin, race, or sexual orientation.

Our CHWs work with members one-on-one to:

• Advocate for and connect them to the correct organizations or people to address their needs • Connect members to resources in their community to help with clothing, shelter, food, childcare, and more • Help members find a primary care provider (PCP) and assist with scheduling their first appointment • Help them understand and access their HUSKY Health benefits • Listen to members’ needs, help create a plan to meet those needs, and guide them along the way to make sure they are able to get the services that are available to them • Meet with members in-person, in an agreed-upon location in their own community • Provide health education that is culturally and linguistically relatable • Support healthcare needs so members can be as healthy as possible • Teach members how to navigate the healthcare system

Partner Spotlight Connecticut Foodshare CHNCT partners with Connecticut Foodshare to improve the health outcomes of underserved populations. To reach HUSKY Health members who utilize the services of Connecticut Foodshare, CHNCT’s Community Engagement Team travels with Connecticut Foodshare mobile pantries when delivering food. CHNCT sets up information tables where Community Engagement Team representatives will provide members one-on-one assistance with understanding available benefits, finding a PCP, scheduling their first appointment, and connecting them with community

resources such as temporary shelter services, assistance with WIC, etc. In support of Connecticut Foodshare’s focus on healthy eating, CHNCT invites our Community Engagement Health Educators to exhibit our nutrition education program, Fat Attack™. This exhibit demonstrates the harmful effects of consuming too much fatty food and suggests healthier alternatives.
