I placed thousands of saline implants. They were not perfect, but for most, they provided a nice option and patient satisfaction was quite high. In 2006, silicone implants were re-approved for use in breast surgery, and the majority of implants placed in the U.S. today are silicone-filled with a silicone shell. The silicone implant options improved over the years, beginning in 2012 with the introduction of more cohesive Sientra breast implants, offered as round and teardrop- shaped. Allergan and Mentor soon followed with similar implants. All these new types of implants contained silicone that was more form-stable, acting like a soft solid (gummy bear) that holds its shape better and stays contained even with a rupture of the implant. Patient outcomes and safety improved with these new implants. We continue to use many of these today. During this time, to allow the use of tear-drop- shaped implants and reduce the complications with implants in general, a textured surface instead of the smooth surface was increasingly used when performing breast augmentation. A very rare association with lymphoma (BIA- ALCL) was identified, and thus most implants placed in the U.S. today are smooth-surfaced (which has no association with lymphoma). I chair the BIA-ALCL committee in the U.S. for

the Aesthetic Society and thus am intimately involved in all aspects of this (including testifying at the FDA panel in March, 2019), but we have never seen a case of BIA-ALCL in our practice at CaloAesthetics. So, today, breast implants are extremely popular and have a very good safety record and high patient satisfaction. But, breast implants are probably not for everyone! We have increasingly offered many alternatives to breast implants for so many of our patients, such as breast lifts, lifts with auto- augmentations (using breast tissue to create an implant effect), and fat grafting. I have lectured extensively on these procedures and this past year produced a Quality Medical Publishing (QMP) video series and published a textbook with Dr. Mays describing all our techniques. Increasingly, we use these advanced techniques for patients choosing to have their breast implants removed. A lot has changed in breast surgery, but one thing has never changed – whether using breast implants, breast lifts, or fat grafting to improve the “look” of the breasts, the most important aspect is creating breasts that help women feel more confident. That is what it is all about!
