A. INITIAL APPOINTMENT 1) Record of Action form

2) Department Director’s recommendation in letter form, and approval signature on Record of Action form. 3) Current curriculum vitae of faculty candidate. The required format can be accessed on the DFCM’s intranet https://fammed2.uc.edu/default.aspx 4) If the applicant will be precepting students or residents, a recommendation in letter form from the Division Directors of the Department of Family & Community Medicine’s Medical Student Education Division or Residency Division. The letter must include specific information regarding contributions to the Department of Family & Community Medicine. 5) Personal statement from candidate that includes a description of the individual’s career goals (not to exceed two pages).

(Initial volunteer appointments – do not require A-126F, Advertising, or an A-900)

B. REAPPOINTMENT 1) Record of Action form
