REGISTRATION PROCEDURES REGISTER online, in person or by mail. Resident registration (mail-in or drop off) begins as soon as you receive your guide. ONLINE • WWW.WILLOWBROOKIL.ORG The Village of Willowbrook Parks and Recreation Department offers online registration through ActiveNet. You can now access the website to register for all Recreation and Active Adult Programs offered by the Village. If you have a current ActiveNet household set up, you are able to register right away. If you are new to ActiveNet, you will need to create a new household before you can register. ActiveNet is a safe and secure system. From our website, click the register online button, and create an account that will be approved by staff. Simply browse to the program you would like, select, and checkout. You will receive an electronic receipt after submitting your secure credit card payment. IN PERSON • Choose the program(s) you wish to register for and fill out the registration form completely, making sure you indicate proper code numbers. Sign the Release/Hold Harmless and Emergency Treatment Form. MAIL • Enclose your check, money order (payable to the Village of Willowbrook) or Visa/Mastercard or Discover number with signature along with the registration form and mail to: Village of Willowbrook, Parks & Recreation Department, 835 Midway Drive, Willowbrook, IL 60527

PROCEDURES 1. All Village of Willowbrook resident registration forms received before February 17 will be held and randomly processed as the first batch of registrations on February 17. Non-resident registration begins February 24. 2. If a program chosen has an opening available, your registration is processed and a receipt confirmation will be mailed within approximately 5 working days. 3. If programs chosen have reached maximum enrollment, your name will be placed on a waiting list, and you will be notified. Every attempt is made to accommodate the persons on these lists. Please be sure to read through the receipt message carefully for an explanation of your registration status. SATISFACTION GUARANTEED REFUND POLICY

PROOF OF RESIDENCY Policies set forth by the Village of Willowbrook Board of Trustees define a resident as anyone living within the boundary of the Village of Willowbrook. Anyone working or operating a business within the Village of Willowbrook boundaries, but living outside the boundaries shall not be considered a resident. We don’t always recognize our neighbors; from time to time, we may ask to see proof of residency when you register for programs in person. All new residents must show proof of residency.

ACCIDENT POLICY The Village of Willowbrook does not provide medical insurance coverage for injuries suffered while participating in Village programs, or while using park facilities. The Village therefore, can assume no responsibility for personal injury while participating in recreation programs or while using park facilities. NON-RESIDENT FEE Part of the cost of programs is assumed with tax dollars. Therefore, those who are not residents of the Village of Willowbrook will be assessed a non-resident fee. Non-residents pay an additional fee per program. Day Camp and Leagues may have different non-resident fee structures.

A full refund will be issued if you are dissatisfied after the 1st class with the program or the program is cancelled by the park district. A prorated withdrawal refund will be made for participants who decide, for whatever personal reason (including injury or illness), that an ongoing program is not right for them. The prorated refund will be based on the number of classes that have occurred at the time the Refund Request form is received by the Parks & Recreation Department. The Director of Parks & Recreation will consider exceptions to this policy due to other extenuating circumstances. The Refund policy excludes trip tickets, adult sports leagues and special events. Please allow two weeks to receive your refund check in the mail.
