WHAT IS INCOME PROTECTION? Income Protection is a Disability Income Insurance Policy that provides a monthly disability benefit payable to an insured employee in the event of a total disability resulting from an off-the-job, covered accident or sickness. THE NEED FOR DISABILITY PROTECTION Have you thought about how you would handle day-to-day living expenses if you could not work for a few weeks or even months due to a disability? » 64% of wage earners believe they have a 2% or less chance of being disabled for 3 months or more during their working career. Disability Income Insurance provides benefits in the event you are disabled.

EMPLOYEE ELIGIBILITY: • A full time employee who works at least 27 hours per week. • Age 17 and up, • Actively at work in your current job for the past thirty (30) days. • A US citizen residing and working in the U.S.

To further enhance your Disability Benefits you may elect one or more of the Optional Riders below: First Hospital Confinement Benefit Pays a Lump Sum Benefit for Insured’s First Hospital Confinement for covered Sickness or Injury during the Calendar Year. Benefit Amount: Depends on the length of Hospital Confinement:1 day $500; 2 days $1,000; 3 days $2,000; 4 days $3,000; 5 days $4,000; 6 days $5,000. The benefit is not cumulative and will not exceed $5,000. Benefit Period: Payable one time per Calendar Year, per insured person. Hospital Confinement includes one continuous Hospital Confinement or several Hospital Confinements for the same or related cause, which are separated by less than 60 days. Hospital Indemnity Rider 1 Pays a Benefit Amount, shown on the policy schedule, while this rider is in force, for a period of confinement, at the direction of and under the care of a physician for the treatment of an Injury or Sickness as defined by the policy to a maximum of 365 days. Hospital Injury Indemnity 1 Pays a Benefit Amount, shown on the policy schedule, while this rider is in force, for a period of confinement, at the direction of and under the care of a physician for the treatment of an Injury as defined by the policy to a maximum of 365 days. Emergency Accident Pays the Specified Benefit for Emergency Care rendered (within 72 hours of the date of the injury) by a Physician in a Hospital Emergency Room or Physician’s office due to a covered Injury. Pays for a maximum of four different covered injuries in a calendar year per insured category (4 for employee, 4 for spouse and 4 for all children, not each child).

Accidental Death & Dismemberment 1 & 2 Provides a death benefit in addition to all other policy benefits if an Insured’s death occurs within 90 (in WA, 365) days of a covered Injury and was a result of a covered Injury. Also provides a lump sum payment for dismemberment or irrevocable loss of sight resulting from a covered Injury within 90 (in WA, 365) days of the covered Injury. Maximum Issue: Employee-$100,000; Spouse-$25,000; Child-$10,000. (In VA, benefit is payable if death occurs within 90 days of the injury causing death irrespective of Total Disability or if death occurs within 365 days of the injury if the injury results in a period of total disability within 30 days of the injury.) Outpatient Sickness Pays the specified Benefit Amount for treatment received in a Physician’s Office or Out-of-Hospital facility due to a covered Sickness or one and one half (1 1/2) times the Benefit Amount purchased for treatment in a Hospital emergency room. Payment will be made for a maximum of four such covered Sicknesses in a Calendar Year per insured category (i.e.; 4 per employee, 4 per spouse, and 4 for all children). Only one benefit amount per sickness. Specified Injury Rider Pays a specific amount for an Injury as indicated within the Rider for: (1) Appliances; (2) Ambulance; (3) Blood/Plasma; (4) Burns; (5) Dislocations; 6) Eye Injuries; (7) Fractures; (8) Ruptured Disk; (9) Tendons; (10) Torn Knee Cartilage; and (11) Gunshot Wound (for Primary Insured only). Building Benefit Rider Amends the Maximum Benefit Period definition in the Policy. The Maximum Benefit period increases based upon the number of Rider Years the Rider has been in force as specified in the rider. The Maximum Benefit Period will not change during the time You are receiving Benefits for Disability. 1 In MD only, riders include a Second Opinion Benefit which will pay a benefit of $50 if any Insured Person is required to obtain an objective second opinion when required by a Hospital utilization review program. They also include an Insured is confined in a Hospital on the date coverage terminates, We will continue to pay the Benefit Amount in accordance with the Rider in effect at the time the Insured’s coverage terminates for the confinement until the earlier of: (1) the date the Insured is discharged from the Hospital; or, (2) 12 months after the date coverage terminates. 2 Extension of Benefits states if an Insured is Injured after the date coverage terminates, We will continue to pay the Benefit Amount in accordance with the Rider in effect at the time the Insured’s coverage terminates if: (1) the Injury occurs while the Insured is covered; and, (2) the loss occurs within 90 days after the Injury. Income Protection is Flexible. You can build your plan by selecting the optional riders that best meet your individual needs.

ELIMINATION PERIOD The Elimination Period is the number of consecutive days at the beginning of your period of total disability for which no benefits are payable. Each new Benefit Period is subject to a new Elimination Period. Only the Elimination Period in effect at the time the loss commences applies. Elimination Period Options - Accident and Sickness 14, 30, 60, 90, 180 or 365* days *In VA, 365 days is not offered. This convenient, affordable disability income plan will provide benefits if you become totally disabled and are unable to work due to a covered injury or illness. Partial Disability The partial disability benefit helps you transition back into full-time work after suffering a disability. If, after being totally disabled, you remain partially disabled and are only able to work 4 hours per day, this plan will still pay you half of your selected monthly benefit for up to three months. Wavier of Premium Premiums are waived after 90 days of Total or Presumptive Disability and while you are receiving benefits. BENEFITS Total Disability
