Accidental Death & Dismemberment 1 & 2 Provides a death benefit in addition to all other policy benefits if an Insured’s death occurs within 90 (in WA, 365) days of a covered Injury and was a result of a covered Injury. Also provides a lump sum payment for dismemberment or irrevocable loss of sight resulting from a covered Injury within 90 (in WA, 365) days of the covered Injury. Maximum Issue: Employee-$100,000; Spouse-$25,000; Child-$10,000. (In VA, benefit is payable if death occurs within 90 days of the injury causing death irrespective of Total Disability or if death occurs within 365 days of the injury if the injury results in a period of total disability within 30 days of the injury.) Outpatient Sickness Pays the specified Benefit Amount for treatment received in a Physician’s Office or Out-of-Hospital facility due to a covered Sickness or one and one half (1 1/2) times the Benefit Amount purchased for treatment in a Hospital emergency room. Payment will be made for a maximum of four such covered Sicknesses in a Calendar Year per insured category (i.e.; 4 per employee, 4 per spouse, and 4 for all children). Only one benefit amount per sickness. Specified Injury Rider Pays a specific amount for an Injury as indicated within the Rider for: (1) Appliances; (2) Ambulance; (3) Blood/Plasma; (4) Burns; (5) Dislocations; 6) Eye Injuries; (7) Fractures; (8) Ruptured Disk; (9) Tendons; (10) Torn Knee Cartilage; and (11) Gunshot Wound (for Primary Insured only). Building Benefit Rider Amends the Maximum Benefit Period definition in the Policy. The Maximum Benefit period increases based upon the number of Rider Years the Rider has been in force as specified in the rider. The Maximum Benefit Period will not change during the time You are receiving Benefits for Disability. 1 In MD only, riders include a Second Opinion Benefit which will pay a benefit of $50 if any Insured Person is required to obtain an objective second opinion when required by a Hospital utilization review program. They also include an Insured is confined in a Hospital on the date coverage terminates, We will continue to pay the Benefit Amount in accordance with the Rider in effect at the time the Insured’s coverage terminates for the confinement until the earlier of: (1) the date the Insured is discharged from the Hospital; or, (2) 12 months after the date coverage terminates. 2 Extension of Benefits states if an Insured is Injured after the date coverage terminates, We will continue to pay the Benefit Amount in accordance with the Rider in effect at the time the Insured’s coverage terminates if: (1) the Injury occurs while the Insured is covered; and, (2) the loss occurs within 90 days after the Injury.
