Health Insurance

The health insurance plan at Levine Academy offers employees and their dependents access to medical and dental insurance benefits and vision care discounts. Regular full-time and part-time employees who work an average of 30 hours a week over 52 weeks are eligible to participate in this plan.

Eligible employees may participate in the health insurance plan subject to the terms and conditions of the agreement between Ann and Nate Levine Academy and the insurance carrier.

You will find details of the health insurance plan in the Summary Plan Description (SPD). When you become eligible, you will receive a SPD and rate information prior to the enrollment date. For questions about the Levine Academy health insurance plan, contact Human Resources. Employees who regularly work 15 hours a week or more are eligible to p articipate in the school’s group short and long term disability insurance and life insurance policies beginning the first day of employment . Details of the coverage are included in each plan’s Summary Plan Document (SPD) which shall be provided to covered employees. Questions regarding these plans should be directed to Human Resources.

The employee’s portion of the premium deduction for health insurance begins on the pay period prior to coverage start date.

This Manual does not contain the complete terms and/or conditions of any of Levine Academy’s current insurance benefit plans. It is intended only to provide general explanations. If there is ever any conflict between the Manual and any documents issued by one of Levine Academy’s insurance carriers, the carrier’s guideline regulations will be regarded as authoritative.

Cobra Benefits

The Federal Consolidated Budget Reconciliation Act (COBRA) gives employees and their qualified beneficiaries the opportunity to continue health insurance cover age under the Levine Academy’s health plan when a “qualifying event” would normally result in the loss of eligibility. Some common qualifying events are resignation, termination of employment, or death of an employee; a reduction in an employee’s hours or leave of absence, divorce or legal separation and a dependent child no longer meeting eligibility requirements. Under COBRA, the employee or beneficiary pays the full cost of coverage at Levine Academy’s group rates plus an administration fee. Levine Academy provides each eligible employee with a written notice describing rights granted under COBRA when the employee becomes eligible for coverage under Levine Academy’s health insurance plan. The notice contains important information about the employee's rights and obligations.

Social Security / Medicare

Levine Academy withholds income tax from all employees' earnings and participates in FICA (Social Security) and Medicare withholding and matching programs as required by law.
