2.4.2 Documentation The Incident Commander is responsible for ensuring accurate and complete documentation is maintained for the incident. The Planning Section is responsible for the collection, management, and storage of all documents. If the Planning Section is not established, this responsibility falls to the Incident Commander. All positions activated in the OSCP, and the ICC are responsible for the completion and submission of required section and position-specific documents.

All ICS documents must be stored and made available for legal, analytical, and/or historical purposes. As a result, it is vital that the information recorded is as accurate as possible with only the facts being stated.
Documents may take the form of:
Text messages

Voicemails
•
Images and video
•
Emails
•
Maps
•
When completing documents for submission:
Print or type all entries.

• Enter date and time on all forms and records. • Fill in all the blanks and use N/A as appropriate.
The Incident Commander, Section Chiefs and the On-Site Commander may assign a Scribe to capture pertinent information.
