Must have a qualifying diagnosis Income under 500% federal poverty level Must have a private insurance policy Qualified referral source
The Access to Care program helps patients and families maintain the continuity of their health care by assisting with premium payments and drug copays. At Hope Charities we understand how critical your medical care is to maintaining your quality of life. We also understand how stressful the threat of losing health insurance coverage or meeting a deductible or co-payment can be when you are already dealing with the weight of a chronic illness. This program is designed to support patients in crisis, ensuring they can continue receiving the medication and care they need without interruption.
