MADD Victim Impact Panel INSTRUCTIONS - PLEASE READ
Registration for the in-person or online program will be found on our website: WWW.MADDVIP.ORG We do not accept registration by phone or email. Pre-registration for the in-person program is highly encouraged. 1. Registration for DAY program starts at 10:15am (program starts at 11:00am and ends at approx. 1:00pm). Doors close and registration ends at 11:00am. No one will be permitted after 11:00am. 2. Registration for EVENING program starts at 6:15pm (program starts at 7:00pm and ends at approx. 9:00pm). Doors close and registration ends at 7:00pm. No one will be permitted after 7:00pm. 3. You must bring a Photo ID and have confirmation email available if you paid by credit card. If you have not paid online, please bring a $75 Money Order ONLY. No other forms of payment will be accepted in person. 4. Please do not arrive until registration begins. You will NOT be allowed inside until that time. 5.

Shelton: Enterprise Corporate Park- 3 Corporate Drive, Shelton Auditorium-1st Floor, parking on site *Do not park in daycare spots* Participants who are not comfortable with fluent English must be accompanied by an interpreter at no extra charge. The interpreter may not be someone already attending the program and they must be over the age of 18.
COURT ORDERED minors under the age of 18 MUST be accompanied by a parent or legal guardian.
NO PERSON UNDER 18 is permitted to attend the program unless COURT ORDERED.
