Who We Are and What We Do Community Health Worker Ambassador Program

Our CHWs help people who struggle with basic needs and use of the healthcare system. They listen to individual needs and concerns and can provide members with the best resources to navigate their situation. Many of our CHWs live and work in the same neighborhoods as our members and have diverse cultural and linguistic backgrounds. Our CHWs serve as ambassadors for the HUSKY Health program. They are committed to building equity through diversity, inclusion, and community engagement efforts. CHWs work to help members reach positive health outcomes regardless of ability, age, cultural background, ethnicity, faith, gender, gender identity, ideology, income, national origin, race, or sexual orientation.

Our CHWs work with members one-on-one to:

• Advocate for, and connect them to, the correct organizations or people to address their needs. • Connect them to resources in their community to help with clothing, shelter, food, childcare, and more. • Help them find a Primary Care Provider (PCP) and assist with scheduling their first appointment. • Help them understand and access their HUSKY Health benefits. • Listen to their needs, help create a plan to meet those needs, and guide them along the way to make sure they can get the services that are available to them. • Meet them in person, in an agreed-upon location in their own community. • Provide health education that is culturally and linguistically relatable. • Support their healthcare needs so they can be as healthy as possible. • Teach them how to navigate the healthcare system.

Community Engagement HUB Program Community Health Network of Connecticut, Inc. ® (CHNCT), on behalf of the HUSKY Health program, has established a network of Community Engagement HUBs throughout the state. A HUB is a neighborhood space where members can get H elp U nderstanding their B enefits. Through these HUBs, CHNCT has established a trusted presence in communities with the highest HUSKY Health member populations, making it easier to reach members who may be unaware of all the community services and resources available to them. Members who visit the HUBs are empowered to make better-informed decisions about their health after being introduced to resources to help meet their basic needs. They can also learn how to live a healthier lifestyle. CHNCT has collaborated with community partners such as Connecticut Foodshare and Workforce Alliance to establish HUB sites where community partners are located. Partners can refer their clients, who are largely comprised of HUSKY Health members, to our information tables at the sites and at community partner events. Examples of events include community meetings, health fairs, parent-teacher nights, and mobile food pantries. HUB events are posted on the HUSKY Health member website and social media pages to let members know when and where they can go to take advantage of these resources. HUBs are generally open for one to two hours at a time and are managed by CHNCT’s Community Health Workers (CHWs). The HUSKY Health program continues to see positive results from establishing HUB sites in community settings that HUSKY Health members frequently visit and trust. Community partners benefit by increasing the number of resources they can offer their

clients, which increases community and member engagement. HUSKY Health members benefit by obtaining information that can help them address Social Determinants of Health (SDOH), including food insecurity, housing, education, and employment. Through resource sharing and working together to impact SDOH, the program improves health equity among HUSKY Health populations throughout Connecticut.

Partner Spotlight Waterbury American Job Center

We are proud to spotlight the Waterbury American Job Center (AJC), as one of CHNCT’s distinguished partners. Our partnership brings the Community Engagement HUB program to this site, enhancing the support available to all who visit.

The Waterbury AJC serves as a central resource for job seekers aiming to upskill, interview with local employers, and access the latest job opportunities. Offering all programs and services at no cost, they collaborate with state, regional, and local organizations to provide comprehensive workforce assistance. This collaboration prepares and trains workers, connecting businesses and industries with skilled talent. At AJC, the Connecticut Department of Labor offers invaluable services such as resume critiques, career counseling, and mock interviewing sessions. Certified professionals in resume writing, interviewing, and career coaching deliver these services, helping over 100 walk-in customers daily meet their employment needs. Our collaboration with the Waterbury AJC allows us to send CHWs to the Waterbury location twice a month, enabling them to connect with members, provide HUSKY Health benefit information, and offer additional community resources. Working together, we are empowering our community through these essential services and partnerships, helping to ensure every individual has access to the support they need for a successful future.

Current Events Healthy Back-to-School Habits: Nutrition and Sleep Tips

As students gear up for a new school year, setting them up for success involves more than just school supplies. A strong start includes a focus on good nutrition and sleep. A balanced diet plays a crucial role in a student’s ability to concentrate and perform well academically. Aim for meals that include a variety of fruits, vegetables, whole grains, and lean proteins. Avoid high-sugar snacks that can lead to energy crashes.

Additionally, adequate sleep is essential for cognitive function and mood stability. Children should get nine to 11 hours of sleep per night, while teenagers need about eight to10 hours. To establish good sleeping habits, create a calming bedtime routine, limit screen time in the evening, and maintain a consistent sleep schedule. These tips will help the students in your life stay healthy and alert throughout the school year!

Public Health Corner Managing Fall Allergies

The fall season can bring a surge in allergy symptoms due to increased exposure to pollen, mold, and dust. To minimize the impact of these allergens, consider taking several proactive steps. Keeping windows closed during high-pollen periods and using air purifiers with High-Efficiency Particulate Absorbing (HEPA) filters can help reduce indoor allergens. Regular cleaning, including vacuuming with a HEPA filter, and washing bedding in hot water, can also help reduce dust and mold. Over-the-counter antihistamines and nasal sprays can offer relief, but it’s important to consult with a healthcare provider for the best treatment plan tailored to individual needs.

Importance of Vaccinations Vaccinations are a cornerstone of public health, helping to prevent serious diseases and keep communities safe. As the new school year begins, it’s essential to ensure that your child’s vaccinations are up to date. Keeping vaccinations current not only protects your child, but also helps prevent the spread of disease within the community. CHW Highlight

We are delighted to highlight Joshua Turner (in photo to left), one of our outstanding CHWs. Joshua joined CHNCT in 2022, as a member of the Health Education team, and has transitioned into the role of a CHW with the Community Health Worker Ambassador Program this past April. Joshua enjoys engaging with the community and coordinating care for those in need. He possesses a deep passion for helping others and believes this experience has facilitated his personal and professional growth, allowing him to learn as much from the community as they do from him. As the newest member of our West Region team, Joshua has been an exceptional addition overall. His commitment and enthusiasm are evident in his work at HUB sites and community events. We look forward to continuing our growth and expansion with Joshua’s dedication leading the way.

DSS has introduced several changes aimed at improving the accessibility and affordability of health services for those enrolled in HUSKY programs. It is crucial for current enrollees to review these updates carefully to understand any modifications to their benefits or coverage. Staying informed about their insurance plans can help them make the most of the services available and avoid any unexpected costs.
