Bad Examples of Labels on Documents
HIS has lately been getting a number of files, where the patient label has not been fixed appropriately or is un- readable. This means that it can not be read by the scan- ner and creates a lot of ad- ditional work for the team.
IMPORTANT REMINDER HIS needs your help...
In order for paper files/assessment documents or reports to be scanned into EMR all staff need to ensure the following: • Put patient label in right upper corner of paper file • Ensure patient label is good quality (no faded ink or smudges) • Bar code is fully visible and not cut off or obscured in any- way
