Introduction Successful transitions are well-planned, thoughtful actions designed to accomplish particular outcomes. Planning and communication are essential to provide a smooth transition and to encourage and sustain collaboration among the families and agencies involved to ensure appropriate services for the child. A transition plan includes several systems and will serve as a guide to participants throughout the transition process.
