LSMS reserves the right to monitor, prohibit, restrict, block, suspend, terminate, delete, or discontinue 5 your access to any Social Media Site, at any time, without notice and for any reason in its sole 6 discretion. LSMS may remove, delete, block, filter or restrict by any other means any materials in 7 LSMS’s sole discretion. You understand and agree that LSMS may disclose your communications and 8 activities with LSMS in response to lawful requests by governmental authorities, including Patriot Act 9 requests, judicial orders, warrants or subpoenas, or for the protection of LSMS rights. You agree that in 10 the event LSMS exercises any of its rights hereunder for any reason, LSMS shall have no liability to 11 you. 12 13 3. By posting any content on any Social Media Site, you grant to LSMS the irrevocable right to 14 reproduce, distribute, publish, display such content and the right to create derivative works from your 15 content, edit or modify such content and use such content for any LSMS purpose. 16 17 4. You shall defend, indemnify, and hold LSMS and its corporate affiliates and their respective officers, 18 directors, employees, contractors, agents, successors and assigns harmless from and against, and shall 19 promptly reimburse them for, any and all losses, claims, damages, settlements, costs, and liabilities of 20 any nature whatsoever (including reasonable attorneys' fees) to which any of them may become subject 21 arising 2 out of, based upon, as a result of, or in any way connected with, your posting of any content to 22 a Social Media Site, any third party claims of infringement or any breach of this Policy. 23 24 5. You expressly acknowledge that you assume all responsibility related to the security, privacy, and 25 confidentiality risks inherent in sending and/or receiving any content via the internet and/or Social 26 Media Sites. By its very nature, a website, Social Media Site, and/or the internet cannot be absolutely 27 protected against intentional or malicious intrusion attempts. LSMS does not control the third party sites 28 and/or the internet over which you may choose to send confidential personal or health information or 29 other content and, therefore, LSMS does not warrant safeguard against any such interceptions or 30 compromises to your information. When posting any content on a Social Media Site, you should think 31 carefully about your own privacy in disclosing detailed or private information about yourself and your 32 family. Please note that this policy may be updated at any time without notice, and each time a user 33 accesses a Social Media Site, the policy will govern usage effective upon engagement. To remain in 34 compliance, LSMS recommends that you review the Policy, as well as the any other website policies, at 35 regular intervals. LSMS INTERIM POLICY MANUAL.

LSMS Physicians Survey

47 Mr. Williams and Ms. Bowen presented the results of the statewide physician survey that was 48 distributed to all licensed physicians in the state. Mr. Williams informed the board that the survey was 49 distributed to approximately 17,000 physicians with 1,049 responding for a participation rate of 6%. 50
