Investigative Report Regardless of the source of the information, a TGRA should always conduct its own investigation to verify the facts alleged and acquire further clarifying information. The TGRA’s investigation should always result in an Investigative Report, identifying the steps followed and the information collected.
Investigative Report An Investigative Report consists of a preliminary statement, findings of fact, opinions, recommendations, and evidentiary exhibits. Exhibits may include documents, surveillance footage, written statements, affidavits, and in some cases, verbatim transcripts or summaries of witness testimony. Other documents that are not probative, that do not tend to prove or disprove a fact, may be attached to the report as enclosures.
