(CHI) soared to new heights in 2023. The organization demonstrated its dedication through increased fundraising endeavors and secured grants that brought transformative change . These strategic advancements highlighted CHI’s commitment to innovation and strengthened its ability to provide comprehensive care as it has done for more than 50 years in the South Florida community.

THE BRODES H. HARTLEY, JR. TEACHING HEALTH CENTER EXPANDS IT’S REACH TO PATIENTS Have you ever had the unique opportunity to receive full care from both seasoned professionals and dedicated residents? The Brodes H. Hartley, Jr. Teaching Health Center at Community Health of South Florida, Inc. (CHI) expanded our ability to bring this experience to patients in 2023. The program unveiled the new family medicine and behavioral health facility at the Doris Ison Health Center in October. The 7,000-square-foot space is equipped with 15 state- of-the-art family medicine exam rooms and 7 consultation rooms for behavioral health services. This clinical expansion not only provides additional access to healthcare for CHI patients but also serves as a great learning environment for the next generation of healthcare professionals.

In this newly renovated department, both teams enjoy designated spaces alongside support staff and nurses. The real gem, however, is the emphasis on residents and providers working closely together, promoting collaboration and opportunities for learning. “If we ever need advice on things like blood pressure medication for our patients, I can easily walk over to family medicine and ask questions to our attending providers,” said psychiatry resident Dr. Ralison Alcius. “It has made the experience very easy.” As the Teaching Health Center continues to grow and evolve, these advancements will better help prepare residents for their future as doctors. This progress not only improves the residents’ training but also improves patient care, reflecting a positive path for CHI.
“I am loving the new space. Its modern design is innovative while keeping us closely connected to our peers and attending providers,” said resident Dr. Marnelle B. Chery. “The learning environment it provides helps us grow and allows us to deliver great patient care.” Designated Institutional Official and Chief of Family Medicine, Dr. Elizabeth Philippe, said the driving force behind this expansion was the commitment to elevate education for the residents and ensure proper care for patients. “The goal was to have an integrated space for the residents where they can see patients and be able to provide primary care,” she said. “We wanted to have a bigger space with more exam rooms so we can accommodate all 35 residents on their clinic days.”

Perspective is important. My team likes to push itself to continue to develop, advance and grow the services and programs at Community Health of South Florida, Inc. (CHI). When you are giving back more than $15 million a year in charitable healthcare, we all want to offer the most, as we recognize people are in need. But I look back at how we started, more than 50 years ago, in 2 double- wide trailers—a dream realized by Doris Ison who rallied the support of volunteer doctors from her church. That perspective helps me realize how far we’ve come. Our team, community, and state and federal stakeholders all worked toward a common goal. Today, we continue to reach and expand that goal with high-quality compassionate healthcare for all, regardless of income, insurance status, or background. We cared for more than 65,000 unduplicated patients in 2023, and that translates to nearly 205,000 visits to our 13 health centers throughout Miami- Dade and Monroe counties. But it also includes our mobile medical van and
mobile dental trailer that have joined forces with our outreach team to make sure we connect with some of the most vulnerable populations and care for them. Community healthcare requires that we are innovative. We can’t use the same template that the average healthcare company might use. We consider the barriers to healthcare that our patients face and create solutions. Some of these solutions included adding cardiology to

our services, as we know many South Florida residents face lack of access to specialist care. It, also, includes supporting a new population health department designed to make sure our patients are coming in to see the doctor and managing their health to help keep them healthy. This team is looking closely at clinical quality measures and improving lives. Our Teaching Health Center celebrates its 10-year anniversary in 2024 and continues to grow and retain the next generation of physicians. That program is also expanding, adding dental and internal medicine to the existing primary care and psychiatry programs. We are also steadfast in improving our state-of-the-art facilities to offer the best to our patients. We have made significant progress in construction and renovation projects such as the new Key Largo Health Center, the Coconut Grove Health Center and the Children’s Crisis Center. We expect Coconut Grove and Key Largo to open this year and the Children’s Crisis Center to be ready in 2025. If you want to know how invested we are in caring for our community, just look at our staff. They believe in our mission and have given back by donating their hard-earned money to the cause. I think that says a lot about this team. Their hearts are big and they also want to see the Children’s Crisis Center become a reality. It’s our future—healing young minds—and we are all in!

CHI Made Breakthroughs in Fundraising Efforts for the Children’s Crisis Center MILESTONES OF COMPASSION:
“The support of the CHI team is phenomenal!” said CHI’s Director of Development, Victoria Castro, who is grateful for every team member’s support. “In the last few years, CHI employees have raised close to $100,000 for the Children’s Crisis Center. That is a big deal!” Ronald McFarlane is CHI’s chef and has worked at CHI since 2014. He said he felt compelled to give back to the children in his community. “During my first week at CHI, I saw a little boy in handcuffs. He was a troubled youth brought to our center by the police for evaluation. I said to myself there must be something that we can do to help these kids,” he said. “They are the future and if we don’t try to help them, who will?” The CHI Giving Society, which honors donor contributions, also grew by strides in 2023, with 136 new inductees. Each donor donated $1,000 or more to CHI since 2021. Throughout 2023, CHI established fruitful collaborations with businesses across South Florida. Macy’s, Kendra Scott, and Chef Adrienne’s Cracked hosted fundraising events for the organization. These collaborations introduced new initiatives like a woman’s mixer in June,

CHI is closer than ever to our goal of opening the doors of South Dade’s first- ever Children’s Crisis Center courtesy of generous donors and innovative fundraising efforts in 2023. The center will be the first of its kind in Southern Miami-Dade County, directly benefiting children suffering from severe mental illness. An impressive $271,000 was raised through diverse initiatives and events, including CHI’s Employee Giving campaign which raised more than $45,000.

designed to strengthen ties with female donors within the community, alongside the 1st Annual Cutler Bay Turkey Trot held in November. Local community members and organizations like the Rotary Club of Perrine-Cutler Ridge/Palmetto Bay united in a momentous walk. This effort resulted in a $9,000 donation generously presented to the President and CEO of CHI, Blake Hall.
CHI is now $258,000 away from completing the $5.5 million Children’s Crisis Center Capital Campaign. The overwhelming community support and dedication have propelled the campaign towards its goal. “I am so happy about the work that we have done,” said Castro. “We are really closing the gap, and we will finish the campaign.”

CHI’S VISIONARY HEALTHCARE LANDSCAPE: A Year of Innovation, Expansion, and Patient-Centric Excellence At CHI, our commitment to enhancing patient care goes beyond traditional healthcare services. In 2023, CHI introduced new technology and services that seamlessly integrated innovative solutions to enhance the patient experience. Early Detection
Mammography Machine The State of Florida awarded CHI with a $500,000 grant to acquire a state-of- the-art 3D mammography machine, a groundbreaking addition to its services. This advanced technology will directly benefit hundreds of women receiving care at CHI, offering lifesaving mammography services for Miami-Dade and Monroe county residents. The implementation of this cutting-edge machine later in 2024 facilitates early detection and treatment of breast cancer. Providing greater access to screenings in the community will reduce late-stage diagnoses, ultimately reducing mortality rates in the South Florida community.
Mobile Medical Van In our mission to enhance healthcare accessibility, CHI’s Mobile Medical Van reached new heights in 2023! Launched in 2020 to deliver exceptional services to patients unable to make the trip to a traditional health center, the medical van became a vital resource for the community. In 2023, the van served and screened more than 511 patients with 1,721 primary care visits in Florida City and Coconut Grove. This continued success showcases CHI’s unwavering commitment to delivering quality healthcare to those in need, ensuring accessibility is never a barrier to well- being. The purchase of the van was funded by Direct Relief and the Health Resources and Service Administration, but inspired by Former Commissioner Dennis Moss. Today, it is partially funded by the Florida Health Department.

Key Largo Health Center Coconut Grove Health Center
The Key Largo Health Center officially began construction in February 2023. Once complete, the center will replace CHI’s current Tavernier Health Center. The 9,000-square-foot property will be the new medical home for patients in the Upper Keys. The center will offer services such as dental, primary care, and behavioral health. The center is expected to open in the Fall of 2024.
Renovation efforts at the Coconut Grove Health Center are nearly complete after continued construction in 2023. The new and improved 6,400 square-feet will provide larger patient rooms, upgraded waiting areas, and cutting-edge medical equipment. With a focus on patient care and advanced technology, CHI is set to offer a modernized experience for people seeking services in the Coconut Grove community. The center is expected to open in the Fall of 2024.
Cardiology Services
In 2023, CHI unveiled cardiology services to patients, marking a significant extension of the health center’s medical program. This initiative allowed CHI to extend its reach to a broader spectrum of patients and addressed the critical need for comprehensive heart health services for its patients. Cardiologists are in high demand and the wait to see one of these specialists in the community can be lengthy. Not to mention many of CHI’s uninsured patients with heart conditions are sometimes unlikely to follow through with a cardiologist due to transportation issues and financial reasons. The newly established cardiology team at CHI provides specialized care for patients facing heart conditions such as uncontrolled blood pressure, heart failure, heart murmurs, and more. This expansion of services aligns with CHI’s mission to offer patients a full range of medical services.
OB/GYN services at Homestead Hospital CHI’s OB/GYN services partnered with Homestead Hospital in 2023 to strengthen support and healthcare for women in the community. With this affiliation, CHI providers direct patients in need of immediate in-patient care, to contracted OB/GYN doctors for deliveries, emergency services, and a variety of obstetric care, at Homestead Hospital. Since May 2023, there have been an average of 65 deliveries a month among CHI patients. This service not only guarantees integrated care but also enables CHI providers to focus on in-house patient care, leading to increased patient capacity. Homestead Hospital staff also receive comprehensive patient records from CHI and are fully equipped with up-to-date information for a seamless transition of care.

In the face of today’s challenges, the demand for comprehensive behavioral health services is more critical than ever. The hardships of modern life, coupled with global crises and economic uncertainties, highlight the urgency of top-notch care for mental health and substance use disorders. Access to high-quality behavioral health services is essential to provide individuals with the necessary support to navigate these challenging times. Community Health of South Florida, Inc. (CHI) is committed to providing high-quality health care, and the $4 million grant awarded by the Substance Abuse and Mental Health Services Administration (SAMHSA) in September, will allow CHI to enhance its services, reducing barriers to access and making healthcare more accessible.
“This is the gold standard for behavioral health services,” said Jean D. Pierre, Executive Vice President, and Chief Behavioral Health Officer. “We’re extremely excited that we will be able to provide these services to the community.” The grant will establish a program at CHI’s Martin Luther King, Jr. Clinica Campesina Health Center and Everglades Health Center. Officially launching in March 2024, it will provide a comprehensive array of mental health, substance use disorders, and primary care screening services to patients, ensuring that services are integrated, person and family-centered.

What will these services include?
Enhanced Patient Support
Mobile Crisis Team Services
Immediate Screening and Risk Assessment Prompt screening and comprehensive risk assessment will be conducted on patients with mental health issues, addictions, and basic primary care needs to address and improve long-term health for patients. This proactive approach aims to address underlying health problems early, promoting better well-being and reducing patient costs associated with behavioral health disorders. The program will offer easy access to care, reducing wait times so those who need services can receive them quickly, regardless of ability to pay or location of residence. Tailored care for active-duty military and veterans will be provided to ensure that they receive the unique health support essential to their treatment. Expanded care coordination will also be incorporated with local primary care providers, hospitals, law enforcement, and more to focus on whole-person health. Peers and family members will be fully integrated into the patient’s care as their involvement will be central in recognizing their recovery.
An around-the-clock mobile crisis team will provide timely assistance to patients, ensuring stabilization in clinically appropriate settings and minimizing trauma. This dedicated team of trained professionals will be committed to delivering tailored support to the patient in a cost- effective manner.

Anticoagulation Program This initiative is for patients who take blood thinning medications. Pharmacists educate patients on anticoagulation therapy, what to eat, INR (International Normalized Ratio) testing and general discussion on therapeutic outcomes. INR is a test used to monitor the medication for people who are taking anticoagulants. CHI’s laboratory conducts the INR test at a minimal cost, as well as Warfarin therapy for free. This includes patient education via workshops every 3rd Tuesday of the month, as an essential component in quality management of
Comprehensive AIDS Resources and Education Services (C.A.R.E.S) CARES provides HIV/AIDs services including testing and counseling, PrEP and PEP medication. It also includes comprehensive care such as medical, dental, prescription drug services, education, medical case management, PrEP Navigation, and more. The program is located at the Doris Ison Health Center, Martin Luther King, Jr. Clinica Clinical Pharmacy Services Pharmacists engage and educate patients by helping review their diagnosis, medications and work closely with them on compliance. They tailor their approach to each patient to better understand any barriers to compliance and encourage optimal health outcomes for patients with chronic diseases such as asthma, diabetes, high blood pressure, hyperlipidemia, Chronic Obstructive Pulmonary Disease (COPD) and others. CHI’s pharmacists and pharmacy educators empower patients to manage the disease as they teach, coach and guide patients. Benefit Enrollment
CHI recognizes that even if a family has health insurance, there may be other obstacles, including transportation, nutrition, food, housing and much more. With the Benefit Enrollment Program, CHI partners with The Children’s Trust to address these issues. It is a holistic approach to helping people to improve the lives of those who care for children. CHI outreach specialists help families find solutions to close care gaps by connecting low income families with children with a wide range of benefits and community based support. Forensic Specialist The court system connects clients to this program so that CHI can assist in assimilating them into the community. Sometimes the client lacks the mental competency required to participate in legal proceedings. CHI’s forensic program helps by providing services such as helping to find housing, case management, linking them to health care, medication management, therapy and more. CHI partners with Thriving Mind South Florida and provides services to work hand-in-hand for the related to the improvement of diabetes control. The team guides patients about managing their disease with positive behavior modifications. For more information contact 305-992-5908 Florida CEAL Diabetes CHI’s Outreach team connects with patients who have diabetes to help educate them on how to better control the disease. Indigent Psychiatric Medication Program This behavioral health program is state sponsored and allows patients at or below the poverty level to receive medication at a minimal cost. Under this program, eligible patients will be provided with important components to successful behavioral health treatment. Class A and Class B patients (individuals at 2% or below the poverty level) qualify for this initiative. For more information contact 305-254-2009 Jessie Trice Preventive Cancer Program CHI’s Community outreach team educates women about the importance of breast and cervical cancer screening. If the women meet criteria for the program they are signed up for a mammogram and pap smear. Medication Assistance Program When a patient can not afford their medication, this program helps get costly drugs discounted to a small fee. A patient must meet financial guidelines to qualify for the program. When the prescription is approved for a reduced rate, patients can pick it up at their CHI pharmacy, or they can request home delivery. The team uses evidence based practices focused on mental health and interventions to better help teachers, parents and the kids manage disruptive behaviors. This is a collaboration between CHI, the University of Miami and The Children’s Trust. The services are based on best practices and recommendations from the American Academy of Pediatrics and Healthy Childcare America. Oral Health Grant CHI’s mobile dental trailer team visits schools and early learning centers to provide oral health education and preventive dental care. The highly trained dentists and licensed dental hygienists utilize top-of-the-line equipment for dental exams, cleanings, varnishes and sealants for uninsured and under-insured children who attend The Children’s Trust affiliated early childcare, after school care and Summer programs. The convenient program allows parents to avoid missing work or a day’s pay. For more information contact 786-400-9064 Project Screen This program focuses on free and low-cost breast and cervical cancer screening. This women’s health initiative is funded by the Florida Department of Health in Miami-Dade County. The screenings are free if the patient meets program eligibility requirements. More than 700 women per year are helped by it at CHI locations. This initiative pays for all exams, tests and treatment, from the biopsy to chemotherapy, for up to five years for those who qualify. School-Based Health If children are healthy, they will learn more, stay in school and cut down on absentee rates. CHI’s school based health program provides efficient cost effective health care services to bridge the gap. This keeps our children strong, healthy and ready to learn. CHI’s team of healthcare professionals serve at 35 school-based health centers in Miami Dade Public Schools. It is funded by The Children’s Trust and Miami Dade Public Schools.The school health team detects and treats medical and behavioral health issues. With its newly added component, telehealth services, students can also receive substance abuse services and obesity intervention. For more information contact 786-594-1081
Specialized Therapeutic Foster Care (STFC) Far too often, children in the foster care system need intensive behavioral health services. The STFC program recruits and trains foster parents to care for the children. It also provides the behavioral health services and case management needed to help these kids. It is funded by Florida Medicaid and the Community-Based Care Lead Agencies. Children placed in specialized therapeutic foster care are victims of domestic violence, parental neglect, abuse and abandonment. Placement in therapeutic foster care helps children learn to live in a healthy family environment and become an active member of the community. In addition, they are taught to develop a healthy attachment to their caregivers and to trust and process emotional traumas. The goal of STFC is to enable the child to be reunited with the natural family, or for placement in a long-term foster home or adoption. For more information contact 305-492-9432 Social Drivers of Health and Insurance Enrollment CHI enrollment specialists help families enroll in health insurance plans and other resource programs that the person might need. The CHI team tracks the application status, linkage and enrollment, and assists the patient in navigating the healthcare system and social care needs. Smoking Cessation Program
This program educates residents and patients about the effects of smoking and secondhand smoke. It also links people to the support program for folks trying to quit.
