Enhanced Patient Support
Mobile Crisis Team Services
Immediate Screening and Risk Assessment Prompt screening and comprehensive risk assessment will be conducted on patients with mental health issues, addictions, and basic primary care needs to address and improve long-term health for patients. This proactive approach aims to address underlying health problems early, promoting better well-being and reducing patient costs associated with behavioral health disorders. The program will offer easy access to care, reducing wait times so those who need services can receive them quickly, regardless of ability to pay or location of residence. Tailored care for active-duty military and veterans will be provided to ensure that they receive the unique health support essential to their treatment. Expanded care coordination will also be incorporated with local primary care providers, hospitals, law enforcement, and more to focus on whole-person health. Peers and family members will be fully integrated into the patient’s care as their involvement will be central in recognizing their recovery.
An around-the-clock mobile crisis team will provide timely assistance to patients, ensuring stabilization in clinically appropriate settings and minimizing trauma. This dedicated team of trained professionals will be committed to delivering tailored support to the patient in a cost- effective manner.
