Anticoagulation Program This initiative is for patients who take blood thinning medications. Pharmacists educate patients on anticoagulation therapy, what to eat, INR (International Normalized Ratio) testing and general discussion on therapeutic outcomes. INR is a test used to monitor the medication for people who are taking anticoagulants. CHI’s laboratory conducts the INR test at a minimal cost, as well as Warfarin therapy for free. Comprehensive AIDS Resources and Education Services (C.A.R.E.S) CARES provides HIV/AIDs services including testing and counseling, PrEP and PEP medication. It also includes comprehensive care such as medical, dental, prescription drug services, education, medical case management, PrEP Navigation, and more. The program is located at the Doris Ison Health Center, Martin Luther King, Jr. Clinica Clinical Pharmacy Services Pharmacists engage and educate patients by helping review their diagnosis, medications and work closely with them on compliance. They tailor their approach to each patient to better understand any barriers to compliance and encourage optimal health outcomes for patients with chronic diseases such as asthma, diabetes, high blood pressure, hyperlipidemia, Chronic Obstructive Pulmonary Disease (COPD) and others. CHI’s pharmacists and pharmacy educators empower patients to manage the disease as they teach, coach and guide patients. Benefit Enrollment
CHI recognizes that even if a family has health insurance, there may be other obstacles, including transportation, nutrition, food, housing and much more. With the Benefit Enrollment Program, CHI partners with The Children’s Trust to address these issues. It is a holistic approach to helping people to improve the lives of those who care for children. CHI outreach specialists help families find solutions to close care gaps by connecting low income families with children with a wide range of benefits and community based support.
