School-Based Health If children are healthy, they will learn more, stay in school and cut down on absentee rates. CHI’s school based health program provides efficient cost effective health care services to bridge the gap. This keeps our children strong, healthy and ready to learn. CHI’s team of healthcare professionals serve at 35 school-based health centers in Miami Dade Public Schools. It is funded by The Children’s Trust and Miami Dade Public Schools.The school health team detects and treats medical and behavioral health issues. With its newly added component, telehealth services, students can also receive substance abuse services and obesity intervention. For more information contact 786-594-1081

Specialized Therapeutic Foster Care (STFC) Far too often, children in the foster care system need intensive behavioral health services. The STFC program recruits and trains foster parents to care for the children. It also provides the behavioral health services and case management needed to help these kids. It is funded by Florida Medicaid and the Community-Based Care Lead Agencies. Children placed in specialized therapeutic foster care are victims of domestic violence, parental neglect, abuse and abandonment. Placement in therapeutic foster care helps children learn to live in a healthy family environment and become an active member of the community. In addition, they are taught to develop a healthy attachment to their caregivers and to trust and process emotional traumas. The goal of STFC is to enable the child to be reunited with the natural family, or for placement in a long-term foster home or adoption. For more information contact 305-492-9432 Social Drivers of Health and Insurance Enrollment CHI enrollment specialists help families enroll in health insurance plans and other resource programs that the person might need. The CHI team tracks the application status, linkage and enrollment, and assists the patient in navigating the healthcare system and social care needs. Smoking Cessation Program

This program educates residents and patients about the effects of smoking and secondhand smoke. It also links people to the support program for folks trying to quit. The person can attend virtually once linked to the program. It is hosted by the University of Miami. For more information contact 305-927-7719
