If your loved one passed away in a healthcare facility, the appropriate authorities, including the chosen funeral home if known, will be notified of the situation. During this emotional and challenging time, the involved emergency and medical personnel will assist you in obtaining the necessary paperwork, including a medical certificate. Additionally, please bring a recent photograph and any clothing you’d like us to use when dressing them for the service.

Verification of eligibility is typically done through the vet- eran's DD-214 or discharge papers. Eligible members include: • Military members on active duty or in the Selected Reserve. • Former military members who served on active duty and departed under honorable condi- tions. • Former military members who completed at least one term of enlistment or period of initial obligated service in the Selected Reserve and departed under honorable conditions. • Former military members discharged from the Selected Reserve due to a disability incurred or aggravated in the line of duty.
