We provide services for adults with Diabetes, Prediabetes and At Risk of Diabetes. • Registered Dietitian • Registered Nurse • Social Worker We offer group workshops on a variety of topics and one- on-one appointments. We will help you gain the tools, skills and confidence to best self-manage your diabetes. We accept Self Referrals.
