Clayton County Community Services Authority, Inc. is a Community Action Agency supported and financed by Federal, State, and Local Funds. Funding for administration is derived from an indirect cost rate negotiated with the Department of Health and Human Services. The Agency is organized and structured in accordance to the laws of the State of Georgia. HOUSING
FOR ATLANTA
F ounded in the early 1900s, HOPE Atlanta is the Metro’s most established agency dedicated to fighting homelessness. While times have changed, our approach has remained steadfast: to end homelessness, one individual at a time. Recognizing an increased need for hunger relief and bold action amid COVID-19 and its economic fallout, Action Ministries joined HOPE Atlanta in 2021. Together, we’re building a comprehensive program to address growing homelessness and hunger needs. Who We Serve We provide individualized assistance, resources, and advocacy for people experiencing (or at risk of experiencing) homelessness and hunger across 28 Georgia counties with special resources for veterans and their families.
A housing-first approach. Stable housing is paramount to a person’s ability to thrive. For each client we serve, our priority is securing housing—both temporary and more permanent solutions. Permanent Supportive Housing Many clients, including those with disabilities or substance abuse issues, need ongoing support after being placed in permanent housing. We provide that support system, assisting with everything from medical appointment transportation to grocery shopping and learning to budget. Special Needs Housing HIV/AIDS cases have continued to spike in Atlanta, particularly among homeless communities. Our dedicated Special Needs Housing team ensures that this population’s needs are met.

CHILDCARE
Our paid staff and volunteer specialty providers are all highly skilled, licensed, health care providers. Our paid medical team meets our patients' primary healthcare needs. Our specialty providers volunteer their time and talents to additionally serve our patients' medical needs. F ayette C.A.R.E. Clinic opened its doors on May 1, 2005 in Fayetteville, GA. Providing only medical care, and open only one evening each week, we began serving the uninsured and lower- income residents in the community. Since that time our organization has grown in significant ways. We've expanded our services, increased our hours of operation, hired more staff, developed relationships with more volunteers and community partners. As a result, we're able to help much more of the underserved population in Fayette County. HEALTHCARE Fayette C.A.R.E. Clinic

Fayette C.A.R.E. Clinic partners with the following organizations to provide quality care to the under-served. We are grateful for these groups which continue to share their experience and support of the work done at the clinic: • Piedmont Fayette Hospital • Northside Hospital • OutPatient Imaging • Ankle & Foot Centers of Georgia • Outcome Medical CONTACT US TODAY VISIT fayettecareclinic.com
Often uninsured and under-served delay medical care because they cannot afford to see a doctor. This puts them at risk of a health crisis due to a lack of preventive care. For those who do manage an initial doctor visit, many are unable to afford needed medications or follow-up visits to resolve their illness. By providing access to a free medical home and the care they need, we help reduce potential health crises for our patients now, and into the future. individuals in our community
