Addendum to Listing Agreement (Compensation)

❏ Sample Form ONLY. Actual form provided by buyer upon submission of offer. This newly required compensation disclosure form is mandated by law to promote transparency in real estate transactions.

MLS Listing Exemption Authorization This form allows us to pre-market / “pocket-list” your property without immediately posting it to the MLS

Private Listing Network Authorization

❏ This allows for us to pre-market the listing "off-market", but through the MLS prior to listing publically

Residential Real Property Disclosure

❏ Not required for new construction, REO / Bank owned, commercial or trust

Radon Disclosure

Not required for units on the 3rd floor and above (Also see required radon testing guidelines packet here)

Lead Based Paint Disclosure

Not required if property was built after 1978 (Also see required disclosure info packet here)

Energy Disclosure (Sample)

❏ Your electric account and gas account numbers are collected on the Listing Agreement, which I’ll use to populate an Energy Disclosure form (if applicable)
