Program of All- Inclusive Care for the Elderly

Provides all-inclusive, comprehensive, and coordinated medical and social services to older adults 55+. Medical care, social services, adult day care, home care and other services. Assists individuals who need support to move from inpatient facilities (nursing facility, hospital) back home or into another community setting. Support connecting to in-home care options, housing coordination, short or longer term case management, help obtaining supplies, home modifications, or medical equipment, and more. A care management program designed to assist older adults to remain in their own homes and communities. Assistance for frail elderly 65+, Medi-Cal eligible. Short-term case management program designed to assist older adults to remain independent and safe at home. Assistance for 60+. Transitions Project helps individuals living in convalescent hospitals and skilled nursing facilities to relocate into their own apartments and homes. Offers programs specifically for those diagnosed with AIDS or HIV. Provides comprehensive services for the entire community, from bill paying to conservator of estate, to help older adults safely manage their finances. Supports transitions home after hospital stays in coastal communities of The Sea Ranch, Stewarts Point, Annapolis, Timber Cove, Cazadero with various needs such as coordinating appts, obtaining needed medical equipment, and other items.

Provides literature to help solve common problems of those in recovery from alcoholism. 24 Hour Help Line which can be contacted during an alcohol related crisis and or to obtain general information regarding Alcoholics Anonymous services and supports. The Family Service Coordination Team provides education, support groups and resource navigation for families with loved ones that are impacted by mental health and/or substance use. Offers family and friends support groups, and outpatient and residential treatment for people with substance abuse. Outpatient and Residential Treatment Programs; family and friends support groups. Provides an affordable, 12-step based residential treatment center for adults seeking recovery from addiction to alcohol and/or drugs. Treatment services include 20 hours per week of group counseling, educational classes, individual counseling, individual learning assignments and family counseling. Drug Abuse Alternatives Center staff are available 24 hours a day at the Orenda Center for alcohol and other drug detoxification services. The Detox Program is designed to provide evaluation and supportive services to assist acutely intoxicated individuals in withdrawing from the effects of alcohol and other drug abuse and plan for continuing recovery. Domestic Violence Hotline

Local Care for Local Elders

Independent Caregiver Support Network

Local Care is a nonprofit support group for local independent caregivers. We meet at the Senior Center once a month to increase skills, make teams, and support each other in providing quality care to our local older adults. NAMI Sonoma County serves residents across Sonoma County with free mental health support, online groups, resources and education. The warm line is available to individuals in crisis without a life threatening emergency and is needing someone to talk to. This program is hosted through Sonoma County. Open 7 days a week, 10am-7pm. We seek to build a community that fosters growth and recovery through connection with peers. We respect individual pathways to wellness and value self direction and mutual support. Offers respite, sliding scale with co-pay, serving caregivers of brain- impaired adults over the age of 18 years.
