Offers arthritis information, a physicians list and referral to local programs. Sonoma County arthritis programs include water and land based exercise classes, public education forums, self-help courses and walking groups. The Family Service Coordination Team provides education, support groups and resource navigation for families with loved ones that are impacted by mental health and/or substance use. Providing palliative, hospice, and skilled home health care. Individual and grief support options. Cystic fibrosis is a genetic disorder that affects the lungs, pancreas, and other organs.
