County of Sonoma Human Services Adult and Aging Division

Offers courses online or in person for Fall prevention. A Matter of Balance is a series of 8 (2-hr) group classes led by trained coaches at different locations throughout Sonoma County. Fall Prevention and Better Balance offers series of 5 (1 hr) classes led by trained coaches online for participation from your own home. Assists in improving the quality of life and health of the Sonoma County community through evidence-based preventive services and self-care classes, emphasizing full participation and decisions affecting health. This program is currently available to patients who have Partnership Only that are having difficulty managing Stage III or IV Cancer, CHF,COPD, and end stage liver disease or neurodegenerative disease. Discounted senior memberships and senior health classes including diabetes prevention program.
