The Sonoma County Aging and Disability Resource Hub is part of the statewide network of Aging and Disability Resource Connec;ons (ADRCs) which put into prac;ce a new, collabora;ve model of streamlined “No Wrong Door” access to services for older adults, people with disabili;es, and their caregivers. The Sonoma County Aging and Disability Resource Hub’s mission is to provide people of all ages, income levels and abili;es with equitable access to informa;on, assistance, and supports needed to live as independently as possible at home. The 2025-2026 edi;on of the Aging and Disability Resource Directory includes services, agencies, and programs which assist older adults, people with disabili;es and their caregivers in their daily lives. The ADRH has made every effort to compile a useful resource directory for the residents of Sonoma County.
