Eldercare Locator
Connects older adults, their families and caregivers with information on older adult services. For services within Sonoma County, call the Aging and Disability Resource Hub at 707-565-INFO (4636). Contact the Eldercare locator for information on services outside of Sonoma County. First time Medi-Cal applicants, Affordable Care Act enrollment, information on Covered CA for current participants. Providing equitable access to information, assistance, and support needed to live as independently as possible at home. Older adults, families and caregivers can connect with a social worker directly to explore services and support specific to their needs. Social workers available by phone, chat or email Mon-Fri 8am to 5pm. Provides free home assessments to make your home more wildfire safe. Partners with Habitat for Humanity to provide repairs to homes that make them more wildfire safe. Please reach out to us at firesafesonomastaff@ gmail.com to see if you qualify and we can get your property assessed and hardened right away! Serving the entire community, provides information and referrals, as well as educational events and workshops on health issues and successful aging. The Northern California Chapter is a lifeline for people living with MS by providing reliable knowledge, engaging and innovative programs and services, financial assistance and a sense of community.
