Form 470 Supplement If a candidate files a Form 470 covering a calendar year in which the candidate is running in an election (i.e., with the declaration of candidacy, in lieu of a first preelection statement, or for the June 30 semi-annual filing) and later receives contributions totaling $2,000 or more, or makes expenditures totaling $2,000 or more, the candidate must file a Form 470 Supplement. When and Where to File the Form 470 Supplement The Form 470 Supplement must be filed within 48 hours of receiving or spending $2,000 or more.
The notification is sent to:
• Secretary of State’s Office;
• Each candidate seeking the same office; and
• City or county clerk, or county registrar of voters, if the candidate is running for a city or county office. The notification must be sent by guaranteed overnight delivery, personal delivery, fax, or email. The candidate must also file a Statement of Organization (Form 410) and begin filing the Recipient Committee Campaign Statement (Form 460). The 24-Hour/10-Day Contribution Report (Form 497) may also be required.
