Candidates are required to account for financial activities associated with their campaign for office. Certain steps must be taken prior to soliciting or receiving any contributions. A candidate may act as his/her own treasurer; however, it is strongly recommended that you appoint a treasurer. Form 501 - Candidate Intention – must be filled with the City Clerk of San Bernardino before soliciting or receiving any contribution from others or before making expenditures from personal funds on behalf of a candidate (excluding personal funds used to pay the fee for statement of qualifications for the sample-ballot pamphlet). Where to file Form 410: San Bernardino City Cler k Department, 201 North E Street, San Bernardino, CA 92401 Monday through Thursday 8 a.m. to 5 p.m. , Friday 8 a.m. to 4 p.m. Form 410 - Statement of Organization, Recipient Committee - must be filed with the Secretary of State within ten (10) days of receiving $2,000 in contributions. This includes monetary contributions, loans, non-monetary (in- kind) contributions, and the personal funds of a candidate or officeholder used in connection with seeking or holding elective office. You will receive written notification from the Secretary of State’s Office assigning an identification number to your committee. Identification numbers are posted on the Secretary of State’s office website at www.sos.ca.gov. The name for all local committees established for an election must include the candidate’s name, office sought and year of the election. An example of this would be: John Doe for San Bernardino City Council 2024. Government Code Section § 84101.5, requires the Secretary of State to charge each qualified recipient committee that files a Statement of Organization (Form 410) $50 annually. The annual fee must be paid no later than January 15 th of each year until the Committee terminates. Committees that form during the last three months of a calendar year must pay the $50 fee within 15 days of filing a Statement of Organization but are not subject to the fee in the following year. Any committee that does not pay the fee is subject to a penalty of $150.
