STATEMENT OF ECONOMIC INTEREST (FORM 700)

Attention Candidates! Forms may be completed online. It’s fast and easy! Simply log into www.fppc.ca.gov, and then click on “View All Forms”

To ensure disclosure of candidates’ financial interest which includes investments, interests in real property, and any income received during the preceding 12 months prior to the date of filing their declaration of candidacy, all candidates are required to file Form 700, Statement of Economic Interests, with the San Bernardino City Clerk at the time nomination papers are filed. Exactly what must be disclosed is specified in Government Code §87000. All candidates for local offices are required to disclose their financial interests during the 12 months prior to the date of filing their declaration of candidacy. The Statements of Economic Interests (Form 700) are a matter of public record and may be inspected and copied when requested. Where to file Form 700: City Clerk Department In Person: 201 North E Street, Suite A, San Bernardino, CA 92401 Monday through Thursday, 8:00 a.m. to 5 p.m. Friday’s 8:00 a.m. to 4:00 p.m. HOW TO ELECTRONIC FILE YOUR FORM 700 CANDIDATE STATEMENT The City of San Bernardino uses an online system provided by NetFile to electronically file Statement of Economic Interests (Form 700) documents. The system is free to use and designed to be easy for the filer. Your data is saved in the system for future filings: net time you need to file, you will only need to edit any changes. This increases the accuracy of your filing and helps to avoid amendments. IMPORTANT ANNOUNCEMENT

NEW USERS How to e-file your Form 700: • Go to Netfile and log into the system.

CURRENT USERS How to e-file your Form 700: • Go to Netfile and log into the system.

• Click on the “New User? Request a Password” clink ON the Form 700 (SEI) section below the log-in form. Submit your email address to receive an email from the system containing a link to request a password. Click on the link to display a web page containing your password. Make sure to record your password. • Log in using your email address and password and begin the Form 700 filing process. • After you log in, click the “Get Help for this Page” button for information and instructions. Each page also includes links to short instructional videos and printed documentation. At the end of the process, you can create a draft document for review. When you are satisfied with the document, e-file your document. • Make sure that on page 1 section 3 check the box that says “Candidate” and insert year “2024”. • You do NOT have to print, sign and submit a paper copy of your e-filed statement.

• If you don’t remember your password, click on the “Lost Your Password” link located under email Address/Password section, enter your email address and click submit an email and be sent to you with a link to create a new password. • Log in using your email address and password and begin the Form 700 filing process. • After you log in, click the “Get Help for this Page” button for information and instructions.
