MEDICAL PLAN
Following is a high-level overview of the coverage available. For complete coverage details, please refer to the Summary Plan Description (SPD).
HSA-Eligible Health Plan The High-Deductible Health Plan (HDHP) gives you the freedom to seek care from the provider of your choice. You will maximize your benefits & reduce your out-of-pocket costs if you choose a provider who participates in the network. In addition, the HDHP comes with a health savings account (HSA) that allows you to save pre-tax dollars* to pay for any qualified health care expenses as defined by the IRS, including most out-of-pocket medical, prescription drug, dental & vision expenses. For a complete list of qualified health care expenses, visit www.irs.gov/pub/irs-pdf/p502.pdf. We are proud to offer you a medical plan that provides comprehensive medical & prescription drug coverage. The plan also offers many resources & tools to help you maintain a healthy lifestyle. Here’s how the plan works:
I mportant: Your contributions, in addition to the company’s contributions, may not exceed the annual IRS limits listed below.
▶ Annual Deductible: You must meet the entire annual deductible before the plan starts to pay for non-preventive medical & prescription drug expenses. NOTE: If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible. ▶ Coinsurance: Once you’ve met the plan’s annual deductible, you are responsible for a percentage of your medical expenses, which is called coinsurance. For example, the plan may pay 80 percent & you may pay 20 percent. ▶ Out-of-Pocket Maximum: Once your deductible & coinsurance add up to the plan’s annual out-of-pock- et maximum, the plan will pay 100 percent of all eligible covered services for the rest of the calendar year. NOTE: If you have other family members in this plan, they have to meet their own out-of-pocket limits until the overall family out-of-pocket limit has been met. ▶ Health Savings Account (HSA): You may contribute to your HSA through pre-tax payroll deductions to help offset your annual deductible and pay for qualified health care expenses. In addition, you will have the opportunity to earn up to $1,000 annually for your HSA by completing various wellness activities submitted through UHC Rewards. To be eligible for the HSA, you cannot be covered through Medicare Part A or Part B programs. See the plan documents for full details.

Employees who work are entitled to take another day during the year, as approved by their department manager. Use your discretion as to the need for skeleton crews on other holidays. Managers, be “heads up” about scheduling PTO time over the Christmas holidays so that we aren’t staffed too lightly such that we aren’t able to service customers.
▶ REMEMBER, TO BE ELIGIBLE FOR HOLIDAY PAY employees must have worked for the company at least 30 consecutive days AND have worked both the last regularly scheduled work day preceding & the first regularly scheduled work day following the holiday, unless the employee arranged for an excused absence IN ADVANCE. IMPORTANT NOTICE
Important Notice
Mental Health Parity Act Per the Mental Health Parity Act, benefits for mental health & substance-use disorder must be treated like benefits for regular medical & surgical care. For example, if there is no limitation on the number of days for inpatient & number of visits for outpatient medical care, then there can be no limitation for mental health & substance-use disorder treatments. As always, treatments must be medically necessary to qualify for coverage. Plan participants should review their plan’s certificate of coverage or benefit document for specific information about coverage, limitations & exclusions for mental health care & substance-use disorder treatments. Women’s Health and Cancer Rights Act On January 1, 1999, a federal law, the Women’s Health & Cancer Rights Act of 1998, became effective, which affects our company plan options. This law requires group health plans that provide coverage for mastectomies (ours does) & to also provide coverage for reconstructive surgery & prostheses following mastectomies. As required under the law, we have included this notice to inform you about it. The law mandates that a participant or eligible beneficiary who is receiving benefits, on or after the law’s effective date (January 1, 1999, for our plan), for a covered mastectomy & who elects breast reconstruction in connection with the mastectomy, will also receive coverage for:
Summary of Benefits and Coverage (SBC) As an employee, the health benefits available to you represent a significant component of your compensation package. The SBC also includes a Glossary of Health Coverage & Medical Terms to help you better understand health care terms used in the SBC. You can obtain a copy of the SBC on the benefit enrollment site. Please note: This guide is intended to provide you with highlights of our benefits program. It is not intended to address all details. Actual benefit coverage is specified in the Summary Plan Descriptions (SPDs). In the event of any differences between this guide & the SPDs, the SPDs will govern.
