Important Notice

Mental Health Parity Act Per the Mental Health Parity Act, benefits for mental health & substance-use disorder must be treated like benefits for regular medical & surgical care. For example, if there is no limitation on the number of days for inpatient & number of visits for outpatient medical care, then there can be no limitation for mental health & substance-use disorder treatments. As always, treatments must be medically necessary to qualify for coverage. Plan participants should review their plan’s certificate of coverage or benefit document for specific information about coverage, limitations & exclusions for mental health care & substance-use disorder treatments. Women’s Health and Cancer Rights Act On January 1, 1999, a federal law, the Women’s Health & Cancer Rights Act of 1998, became effective, which affects our company plan options. This law requires group health plans that provide coverage for mastectomies (ours does) & to also provide coverage for reconstructive surgery & prostheses following mastectomies. As required under the law, we have included this notice to inform you about it. The law mandates that a participant or eligible beneficiary who is receiving benefits, on or after the law’s effective date (January 1, 1999, for our plan), for a covered mastectomy & who elects breast reconstruction in connection with the mastectomy, will also receive coverage for:

To help you make an informed choice, the company makes available a Sum- mary of Benefits and Coverage (SBC), which summarizes import- ant information about our health coverage in a standard format, to help you compare across options. The SBC also includes a Glossary of Health Coverage & Medical Terms to help you better understand health care terms used in the SBC. You can obtain a copy of the SBC on the benefit enrollment site. Please note: This guide is intended to provide you with highlights of our benefits program. It is not intended to address all details. Actual benefit coverage is specified in the Summary Plan Descriptions (SPDs). In the event of any differences between this guide & the SPDs, the SPDs will govern.

TECHNICIAN PERKS

Tuition Reimbursement Program Wolter will reimburse your student loans up to $10,000. This program is for full-time technicians who have completed a 1 or 2-year technical college program within the last 2 years. Interest – Free Tool Loans Technicians can borrow up to $3,000 to purchase tools. Loans are interest free & repaid via payroll deduction. Available to journeymen technicians after 30 days, or apprentice technicians after 90 days. Rent – To – Own Tool Program Apprentice hires have the option to “rent to own” a starter set of tools. The company will purchase a starter tool set up to $2,850 in value for the apprentice to use & the apprentice rents the tools until paid in full. Payment amount is based on the value of the tool set. Tool Insurance Wolter pays the cost of insurance to cover our technicians’ tools against damage or theft.Technicians must have a tool inventory list on file to receive this benefit. Safe Driver Award Program Wolter recognizes employees who drive safely. Employees who drive company vehicles & are accident free for a full calendar year receive an annual safe driver bonus. Reward amounts are based on position.

Safety Shoe Program Wolter subsidizes the cost of safety shoes. Technicians & truck drivers will be reimbursed up to $150 towards the cost of new safety shoes, once per rolling 12 months. Available after 90 days of service. Prescription Safety Glasses Program Wolter subsidizes the cost of prescription safety glasses. We will pay $150 towards the cost of glasses once every two years. Available after 90 days of service. Free Training All technicians are provided 80+ hours of free technical training per year. Both e-learning and classroom training are available.

▶ All stages of reconstruction of the breast on which the mastectomy has been performed; ▶ Surgery & reconstruction of the other breast to produce a symmetrical appearance; & ▶ Prostheses & treatment of physical complications of all stages of mastectomy, including lymphedemas.

Free Uniforms We provide uniforms & uniform cleaning at no cost.

This coverage will be provided in consultation with the patient & the patient’s attending physician & will be subject to the same annual deductible, coinsurance and/or copayment provisions otherwise applicable under the Plan.
