I have fallen in the last 6 months. People who have fallen once are likely to fall again. Yes (2) No (0) I use or have been advised to use a cane or walker to get around safely. People who have been advised to use a cane or walker may already be more likely to fall. Yes (1) No (0) Sometimes I feel unsteady when I am walking. Unsteadiness or needing su pport while walking are signs of poor balance. Yes (1) No (0) I steady myself by holding onto furniture when walking at home. This is also a sign of poor balance. Yes (1) No (0) I am worried about falling. People who are worried about falling are more likely to fall. Yes (1) No (0) I need to push with my hands to stand up from a chair. This is a sign of weak leg muscles, a major reason for falling. Yes (1) No (0) I have some trouble stepping up onto a curb. This is also a sign of weak leg muscles. Yes (1) No (0) I often have to rush to the toilet. Rushing to the bathroom, especially at night, increases your chance of falling. Yes (1) No (0) I have lost some feeling in my feet. Numbness in your feet can cause stumbles and lead to falls. Yes (1) No (0) I take medicine that sometimes makes me feel light-headed or more tired than usual. Side effects from medicines can sometimes increase your chance of falling. Yes (1) No (0) I take medicine to help me sleep or improve my mood. These medicines can sometimes increase your chance of falling. Yes (1) No (0) I often feel sad or depressed. Symptoms of depression, such as not feeling well or feeling slowed down, are linked to falls. Yes (1) No (0) TOTAL Add up the number of points for each “yes” answer. If you scored 4 points or more, you may be at risk for falling. Discuss this newsletter with your doctor. This checklist was developed by the Greater Los Angeles VA Geriatric Research Education Clinical Center and affiliates and is a validated fall risk self-assessment tool (Rubenstein et al. My experience at Fyzical has changed my expectations of what physical therapy is and how impactful it can be! “I came to Fyzical Therapy and Balance Centers with very low expectations of being helped in order to restore any quality of life. I had been in and out of physical therapy clinics from 1984 to 2010 after sustaining life changing injuries from a car accident. Over the years, I have dreaded going to therapy knowing I would leave in more pain with no end in site. I had avoided therapy for over 9 years and when my family physician suggested therapy again I looked at him and said that It would not help and actually felt hopeless with that response. I decided after a month to try Fyzical Therapy and Balance Centers after reading reviews and knew I needed to try something besides what was occurring at the time. From my first visit I felt that I was going to be helped. I was thoroughly evaluated and had the chance to ask questions and knew that the therapist understood my apprehensions about therapy, but made me feel that I was more than just a patient, but someone who desperately was looking for help to restore my life. After seeing Jeremiah and Julio, I now look forward to attending my sessions. I have made amazing changes and my quality of life continues to improve. I am experiencing less pain, can play with my grandkids, garden and spend time with family without worrying if I will be in so much pain. The professional care and passion they have shown has given me confidence in my physical abilities and that I can resume a normal life. I can only say that my experience at Fyzical has changed my expectations of what physical therapy is and how impactful it can be on one’s life. I now wake up ready to take on the day!!
