Benefits Enrollment Checklist
If you do not complete enrollment, you will be defaulted into standard coverage (UMP Classic with surcharges, Uniform Dental, basic life insurance) and may not make changes until open enrollment. Retirement default is CWURP.
Enrollment must be completed within 31 days of eligibility (start date) to avoid default coverage. Health Benefits • Medical, Dental, Vision: Enroll Online at Benefits 24/7 Additional Benefits • Life Insurance (MetLife) • Long-Term Disability (LTD) Retirement • Retirement Status Verification • Retirement Plan Election • Faculty: CWURP Election Form P If choosing TRS 3, submit DRS HERP Enrollment Form also
