Complete Medical Report Form found in ERNIE > Tools > Medical/Health Portal > My Forms. Provide proof of (2) measles, mumps and rubella immunizations, (3) hepatitis B injections and a meningococcal vaccine. This must be on file in the Wellness Center prior to Orientation to avoid a registration hold, housing check-in hold, inability to pay bills and other holds. Health insurance is mandatory. All students are automatically enrolled in the university policy, and the cost will be added to your student account. If you have comparable health insurance and want to opt out, you must complete a waiver through ERNIE. Watch your Student Homepage Task tiles for “Health Insurance Waiver/Election” to appear as an action item in July.

Bring course outlines/syllabi for transfer courses if previously requested.

Register your vehicles beginning in mid-August (see page 11).

For students under age 18 — A parent/guardian must sign required consent forms for medical treatment, counseling services and participation in student activities. Forms will be sent directly to the student via email prior to the start of the semester.
