 Complete the free Application for Admission (http://www.lbwcc.edu/student-services/admissions)  Submit the Free Application for Federal Student Aid (FAFSA) online at www.fafsa.gov and be sure to enter the LBW School Code (008988)  Request all transcripts be sent to LBW  High school transcript  Copy of GED  Previous college transcript, if transferring  Submit your ACT or SAT scores to the Student Services Office

* The DMS Program does require 19 prerequisite credit hours to be completed prior to enrolling in the program. ** Upon successful completion of the first 3 semesters of the Associate of Applied Science Nursing Track 1 Program students will be awarded a certificate in Practical Nursing and will be eligible to apply to take the NCLEX-PN. *** The Nursing (RN) Track 2 enables the Licensed Practical Nurse (LPN) or the Licensed Paramedic to complete the Associate Degree Nursing Program. This track does require twenty-one prerequisite credit hours to be completed prior to enrolling in the program.
