Alternates:
(1) Alternate No. 1- OCIP - See bid form. Deduct $270,000 (included in contract)
(2) Alternate No. 2 – Provide Geopiers on 15’ centers under the west half of the building underneath the slab on grade. Affected area is between columns 8.3 and 12.9 and column line Q and E. Includes 155 Geopiers. Add $121,000 (3) Alternate No. 3 – In lieu of the ground floor on the east side of the building being built with caissons, grade beams, caisson caps, thickened reinforced slab, and the east perimeter exterior wall on column line 19 being +/-16’; furnish and install a conventional slab, standard frost wall on column line 19, Geopiers, and concrete piers similar to that as shown on the west side of the building. Deduct $ NOT USED
(4) Alternate No. 4 –Provide Geopiers on 15’ centers under the east half of the building if alternate number 3 above is accepted. Add $ NOT USED
(5) Alternate No. 5-Omit providing the concrete work at the healing courtyard as shown on page L04. Price includes flat work, underground concrete basin, seat wall,; precast concrete copping with flashing/caulking/stainless steel anchorage per details 3, 4, and 5 including the aggregate under the wall. (Aggregate under landscape items detail 2 will be by others). Work will be done fall of 2009. Deduct $30,000 (6) Alternate No. 6-Omit Providing the concrete work at the seasons garden as shown on page L05. Price includes flat work, concrete curb with stone, concrete paving and stone per detail 5 (wearing surface by others). Work will be done in fall of 09’. Deduct $14,500
(7) Alternate No. 7-Provide detail 6 page S5004 complete including insulation and reinforced concrete at each column as indicated. Base bid is raw structural steel column, no work for this subcontractor. Add $26,000
(8) Alternate No. 8- Provide 10mil visqueen vapor barrier under the slabs on grade, instead of the Under-Slab Gas and Water Vapor Barriers specified.(Note the 4’ of horizontal insulation At the patient bed tower is to remain). Deduct $187,000
(9) Alternate No. 9- Delete all caissons except for the exterior canopy and the twelve (12) under the patient bed tower. Deduct $350,000
(10) Alternate No. 10- Omit the helical tieback system from the scope of work. Deduct 1,058,500.00
(11) ABC Design Group Alternates:
(a) HDG Alt. L-1; Add monument sign foundations including excavation, stone aggregate, backfill, etc. Add $52,000 (b) HDG Alt. L-2; Add integral color to the concrete walk from hospital main entry north along MOB 1 and the bus stop. Add $4,000 (c) HDG Alt. L-3; Add integral color to the concrete walk around the south end and east sides of the main entry drop off. Add $7,000 (d) HDG Alt. L-4; NOT USED (e) HDG Alt. L-5; Main entry enlargement A- Add medium sandblast to the colored concrete. Add $30,000 (f) HDG Alt. L-6; Main entry enlargement B- Install unit paving and colored concrete band at the hospital main entry in lieu of colored concrete walk. Includes stone sub-base, concrete sub- base, weeps, etc. per details 1 and 2 on page L02 (sand leveling bed and pavers by others) Add $7,000 (g) HDG Alt. L-7; Add integral color to concrete pavement in the fire lane and integral color to concrete walks. Add $28,700 (h) HDG Alt. L-8; NOT USED (i) HDG Alt. L-9; Add integral color to the concrete walk in the main entry island. Add $2,000
