This Project Manual contains the Bidding and Contract Requirements for Acme Health System - Acme Hospital project in Acme, IL . Review of this Manual is important in fulfilling the Contract Requirements. Any questions regarding this Manual or any of the Bidding or Contract Requirements should be directed to the Leopardo Companies Inc.. List all consultants working on the project below.

will be considered part of the Bidding and Contract Documents. [IF YOU ARE NOT DOING A PREBID CONFRENCE, THEN STAE THAT THEIR WILL NOT BE PREBID. THIS WILL INSURE CONTINUATY WITH OTHER SECTIONS OF THE MANUAL] 10. [INCLUDE INFORMATION REGARDING ANY SPECIAL OUTSIDE PRINTING ARRANGEMENTS] A deposit of [$______________INSERT AMOUNT] REVISE THIS SECTION IF YOU ARE FURNISHING A COMPLETE SET OF BID DOCUMENTS AND ADDNEDUMS AT NO COST. INDICATE THAT THEY MAY PURCHASE DIRECTLY FROM THE PRINTER ADDITIONAL SETS IF REQUIRED. per document set is required. Deposit checks should be made payable to the Leopardo Companies Inc.. Each Bidder shall provide its shipper number for shipping fees if the Bidder desires to have plans sent by ground or air transportation. More than one set is available upon payment of printing and shipping costs. Deposits will be refunded upon return of the Bidding Documents to the Leopardo Companies Inc. within twenty (20) days after opening of the Bid Proposals provided the Bidding Documents are complete, in clean and usable condition, and free of marks or other defacements. Successful Bidders shall retain their set of Bidding Documents and their deposits will be refunded upon execution of the Agreement. 11. Bidders must be pre-qualified [OMIT THIS SECTION IF NOT REQUIRED OR EDIT ACCORDINGLY]for this bid package by the Owner and Leopardo Companies Inc. before document distribution or receipt of bids per the Instructions to Bidders. The Owner and Leopardo Companies Inc. reserve the right to request qualification forms or additional information from any Bidder before issuing Bidding Documents, receiving Bid Proposals or awarding the Agreement. 12. Bid Proposals shall be on forms furnished by [Leopardo Companies Inc. in Section 00400 or A/E]. Bidders will be required to submit with their Bid Proposals a Bid Security by a qualified surety authorized to do business in the state where the Project is located, [NOTE: BID SECURITY IS OFTEN REQUIRED UNDER PUBLIC BIDDING STATUTES. DOUBLE CHECK BEFORE DECIDING NOT TO REQUIRE BID SECURITY], an OSHA Form 300 for the most recent completed year, their worker's compensation Experience Modification Rate (EMR) factor, and any other information required in the Instructions to Bidders. Bid Proposals will be received by 5200 Prairie Stone Parkway, Hoffman Estates, Il. 60192 delivery or mail, to the attention of ____________ by _____ p.m. local time on _____. 3. Accepted Bidders will be required, as a condition precedent to award of Contract, to furnish in the amount of 100% of the contract price, satisfactory Performance Bond and Payment Bond and Certificates of Insurance as required in the Project Manual. [NOTE: CHECK WITH THE OWNER AND PROJECT EXECUTIVE AS TO WHETHER BOND REQUIREMENTS WILL BE MODIFIED FOR THIS PROJECT] 4. [PROJECT DESCRIPTION]
13. The successful Bidder(s) will be required to enter into an agreement with Leopardo Companies Inc. (VERIFY WITH OWNER)] on the Agreement Form identified in Section 00500 of the Project Manual.
5. Unless otherwise specifically set forth in Section 00880 of the Project Manual, this Project is subject to state sales and/or use taxes and Bidder is required to include such taxes in its Bid Proposal.
14. The right to reject any or all Bid Proposals, either in whole or in part, or to waive any informalities or irregularities therein is reserved by the Owner [and/or] Leopardo Companies Inc.. [NOTE: FOR SOME PUBLIC PROJECTS (E.G., PUBLIC SCHOOL DISTRICTS), ONLY THE OWNER CAN ACCEPT/REJECT BIDS. MAKE SURE THIS SECTION COMPLIES WITH THE APPLICABLE PUBLIC BIDDING STATUTE]
6. Leopardo Companies Inc. has been contracted by the Owner in the capacity of Construction Manager for the Project, and as such has the rights and obligations set forth in its contract with the Owner for those services, and shall act as representative of the Owner to the extent required/allowed under its Owner contract. 7.
