ii. Adjacent doors shall be kept closed and sealed with masking or duct tape. Doors that are in use must either have a seal or a damp towel placed at the threshold to minimize dust dispersion into occupied areas. iii. Negative pressure shall be maintained by the [insert Contractor / Bid Category] in all Work areas at all times – supply vents must be shut off and return vents must be filtered [Verify with Owner type of filter required] . Additional exhaust fans shall be placed in exterior windows if applicable – these fans shall also be filtered if exhausted air is near other operable windows serving occupied areas, or near air intakes. iv. Use sticky mats or damp carpets at thresholds to and from construction areas which shall be maintained by the [insert Contractor / Bid Category]. v. All Subcontractors shall transport construction debris in tightly covered containers either wrapped in visqueen or using solid sealed lids. Trash containers and their wheels must be wiped clean upon exit from construction area through an occupied area. Debris should be transported during the lowest activity period (e.g. at the beginning and end of the day). Debris on the outside of the building shall be handled in a manner to prevent dust dispersal. vi. All due care should be taken in delivering construction materials so as not to spread dust before or while entering the construction area (e.g. drywall or powder materials). vii. Vacuums must be used to collect dust during cutting and sanding, and/or water misting of Work surfaces shall be performed to control dust while cutting and sanding. viii. Prior to dismantling barriers, the construction area must first be thoroughly cleaned. Barriers shall be taken down in a manner that does not allow the dirt/dust on the barriers to become airborne. Misting of interior of barrier walls and floor with water is required prior to dismantling. Wet mopping of the portion of floor beneath the barrier is required after removal. 3. Class 3 – Class 3 Work is long-term construction Work (longer than two months. In addition to Class 1 and Class 2 requirements, all Subcontractors shall perform the following: i. Hard (drywall or other approved material) barriers are required and shall be constructed by the [insert Contractor / Bid Category]. Verify Interim Life Safety measures are not compromised due to barrier installation. These barriers shall be constructed from floor to above the ceiling on the public/occupied side. On the public side of the barriers all joints should be mudded and taped, and the top and base should either be caulked to the floor and ceiling, and/or wall base be installed to prevent flaking of drywall dust. [Verify with Owner if long-term barriers should be painted for aesthetic reasons.] The Owner and/or BMC will determine barrier locations prior to commencing Work. ii. Interior vestibules or anterooms (either visqueen or drywall as appropriate and as approved by BMC) shall be constructed by the [insert Contractor / Bid Category] at construction entry points from public/occupied areas. These vestibules are considered buffer zones so the public corridor/area is not immediately exposed to the construction site and associated airborne dust. iii. [insert Contractor / Bid Category] shall continuously review the interior and exterior perimeter of the construction area for miscellaneous openings, penetrations, registers, doors, windows, or other types of openings and completely seal openings with the appropriate barriers to confine construction dust, air and air pollution from occupied areas. iv. Equipment locations and equipment type should be reviewed with Leopardo Companies Inc. to ensure exhaust, fumes or exterior dust does not interfere with any air intakes or public entrances. v. Negative air pressure shall be maintained at all times by the [insert Contractor / Bid Category] . Seal off completely or filter air returns – [Verify with Owner types of
filters that will be required – 50% HEPA filters should be considered a minimum standard] . Filters should be cleaned or changed when the back side is visibly dirty. vi. Use sticky mats or damp carpets at construction entry points from occupied areas – these should be placed and monitored at least once daily by the [insert Contractor / Bid Category]. Mats should be changed regularly and whenever they are covered with dirt. vii. Public corridors, patient areas and elevators used for construction access shall be monitored for excessive dust and should be mopped, vacuumed, etc. several times daily by the [insert Contractor / Bid Category]. viii. A specific traffic pattern for material deliveries and construction worker access will be designated by Leopardo Companies Inc. and shall be adhered to by all Subcontractors and their Subordinate parties. ix. The Work area shall be cleaned frequently by each Subcontractor to minimize dust. Each Subcontractor shall avoid using equipment that stirs up dust or dry sweeping and shall be fully responsible and liable for any dust that is spread or stired up on the Project site. x. When workers leave the Work area, Subcontractor shall ensure that their clothes, shoes and hair are not excessively dusty. At times when the work site is very dusty, workers must either vacuum themselves off using a HEPA vacuum cleaner, or mist themselves, their shoes or hats with water to clean off excessive dust. [Verify with Owner – for extreme circumstances, only] Cloth or paper coveralls and shoe covers shall be worn on the construction site by all workers and shall be removed upon departure from the Work site. xi. Prior to barrier removal, the [insert Contractor / Bid Category] shall thoroughly clean the Work site. Temporary visqueen barriers shall be required when dismantling drywall barriers. 4. Class 4 – Class 4 work is long-term and highly sensitive Work. In addition to Class 1, Class 2 & Class 3 requirements: i. The [insert Contractor / Bid Category] shall remove or add filters to the HVAC system in areas where Work is performed to prevent contamination of duct systems including at exterior air intakes. [Verify with Owner type of filters required – 80% or higher HEPA filters shall be considered the minimum requirement] ii. The [HVAC Contractor or other] shall maintain negative air pressure within the work site utilizing HEPA equipped air filtration units with the exhaust air going outside of the Work area – NOT back to any air handling units. If exhaust air is directed outside the building, it does not have to be HEPA filtered unless the exhaust is within 100’ of an air intake, operable window or main / public entrance. iii. The [insert Contractor / Bid Category] shall verify that negative pressure exists within the Work area by utilizing a differential pressure gauge or flutter strip in the wall separating the work area from the occupied areas. [insert Contractor / Bid Category Foreman or other party] shall verify negative pressure every day that Work is occurring and a daily log shall be maintained. iv. [Verify with Owner] Utilize a HEPA filter unit immediately outside the entrance to the Work area to minimize the spread of airborne dust. v. All diesel-fueled equipment must be equipped with scrubbers and/or use, at minimum, a 5% soy-based bio-fuel to minimize the smell of diesel.

B. In addition to the requirements listed above, special attention is required to the following for all Subcontractors:

Matrix of Precautions for Construction & Renovation
1. Noise and Vibration – each Subcontractor shall review the type of equipment or methods of operation in consideration of adjacent occupied areas that could be adversely affected by noise or vibration, and shall use best efforts to minimize any noise or vibration affecting such areas. Coordinate the scheduling of any work causing excessive noise and/or vibration with Leopardo Companies Inc. Co. prior to beginning work. 2. Water Damage / Leaks Prevention – the [insert Contractor / Bid Category] shall pay special attention to any possibility of leaks or water damage due to construction Work and its impact on surrounding occupied areas. 3. Exhaust & Fumes - for Work involving hazardous fumes including, but not limited to, adhesives, paints/primers, grinding, saw cutting, equipment exhaust, or welding, each Subcontractor shall review alternate materials or methods of Work that may be required depending on adjacent occupied areas. Construction activity is not permitted within [twenty-five (25) feet - verify with Owner] of existing fresh air intakes. 4. All of Owner’s security measures must be strictly followed by all Subcontractors and their Subordinate Parties including, but not limited to, rules about limited construction access at specified doors/entry points, wearing proper identification, and emergency procedures. 5. All shut-downs for electrical, plumbing, mechanical, fire protection or any other service shall be scheduled with the Owner through Leopardo Companies Inc. at least [48 hours – verify with Owner] prior to the shut down, and affected occupied areas must be reviewed with the Owner at that time. Should an unscheduled shut-down or interruption of services take place, Leopardo Companies Inc. shall be notified immediately and the Subcontractor shall be required to remedy the situation immediately and shall review the situation with the Owner and Leopardo Companies Inc. after service is restored. 6. If modifications to the existing ventilation system are necessary, Subcontractor shall verify that existing systems as modified can produce the proper air exchange rates and pressure required in critical areas, and that air is not being directly circulated from construction areas into other occupied/patient care areas and shall document such verification to the Owner through Leopardo Companies Inc.. 7. Subcontractor is required to review its proposed cleaning techniques for exterior façade of existing buildings, ramps and walks with Leopardo Companies Inc. and the Owner. Water blasting techniques =shall be used (instead of sandblasting) near air intakes, mechanical rooms or patient care areas. 8. Each Subcontractor shall provide for its Subordinate Parties protective apparel as required (coveralls, foot gear and head gear) suitable for use by construction personnel and authorized visitors when leaving Work areas and passing through adjacent occupied areas. 9. When finished Work is installed, each Subcontractor shall cover and protect its own absorbent materials (carpets, fabrics, etc.) to assure that such materials do not absorb excessive dust and/or debris before an area is finally cleaned and turned over to the Owner for occupancy.
This form should be completed when planning any demolition, construction or renovation work as referenced in the JCAHO Standard EC.3.2.1.
