Instructor Qualification Standard / V2 DRAFT 2025-08-26_
• Serve as documentary evidence of successful training and support the upload of records to WINDA
Training providers must retain a copy (physical or electronic) of the completed form for each participant and for every module attended and make these records available to GWO upon request. Forms may be digitised or adapted for use with digital systems, provided all required content is preserved.
Training providers must also have a documented appeals procedure in place for participants who do not pass a module. This procedure must be made available to participants prior to the start of training.
Use of the mandatory assessment form does not restrict training providers from developing and using additional assessment tools or supporting materials as part of their broader assessment strategy. Any additional tools must align with the principles of assessment and rules of evidence, and contribute to valid, fair, reliable, and flexible assessment outcomes.
All assessment documentation must be retained and made available for audit or verification, in accordance with GWO Requirements for Training Providers.
