This standard is closely related to others in section 5: Provision of Care and Record Management, but it focuses on the expectation that the IDG maintains frequent oversight of the care of each hospice patient demonstrating attention to changes in their condition and making appropriate adjustments in the plan of care. ■ Audit IDG review documentation for participation of all IDG members, including collaboration with the attending physician. ■ Compare IDG reviews with the plan of care to validate that updates are made and communicated to those providing services. ■ Establish protocols to ensure the plan of care is reviewed at least every 15 days. ■ Train staff to revise the plan of care to discontinue any resolved problems, goals and interventions. ■ Train staff to update the plan of care with goals and interventions for newly identified problems.
