The intent of this standard is patient safety and continuity of care. A complete medication profile consists of:
Nerd Newbies (understand the requirement)
■ All current medications. ■ Date prescribed or taken. ■ Medication name. ■ Dose. ■ Route. ■ Frequency. ■ Date discontinued. ■ Allergies.
Overview of the requirement: Client/patient medications, both prescription and nonprescription, are reviewed on an ongoing basis by a registered nurse (RN), pharmacist, or licensed skilled professional. Comment on deficiencies: Compliance is assessed through observation and review of policies, procedures, and client/patient records. Deficiencies indicated missing documentation in the medication profile. Examples of ACHC Surveyor findings: ■ Medication profile did not include a frequency for administration of IV therapies. ■ Medication frequency was documented “PRN” without additional parameters. ■ Medication profile did not include a diluent or mixing instructions for polyethylene glycol oral powder listed. Other medications were missing dosage and frequency. ■ Medication profile does not include all medications patient is currently taking. It only includes medications provided by the agency’s pharmacy.
■ Audit client/patient charts for completeness of the information for each medication on the profile.
Nerd Apprentices (audit for excellence)
■ Adopt a written policy and procedure for maintaining a current medication profile for each client/patient. ٝ Require physical review of all medications present in the home at each visit. Interview patients and caregivers regarding whether all items present are being taken, including the frequency, dose, and route of administration. ٝ .Highlight any variance for the RN or pharmacist who conducts the regular review.
