e. Payment receipt of water analysis fee, as established in the Regulation of Fees for Services provided by the National Service of Health and Food Safety and its validity. f. Copy of the Property Title if the establishment is not owned, a duly authenticated copy of the lease agreement or a signed and stamped document indicating the landlord authorizes the lessee to make the improvements or remedial actions to the building infrastructure, according to the observations found during the SENASA inspection visit, if required. g. Affidavit authenticated by Notary Public issued by the Legal Representative of the establishment, indicating that the company complies with sanitary hygienic aspects. h. Plan or sketch map of the establishment areas, and flow diagram of operations or process. i. Affidavit of the presumed amount to be processed, produced, or imported in kilograms or liters in a year. j. List of providers. * The National Service of Health and Food Safety (SENASA) may request further detail of technical information presented by the interested parties. VALIDITY The Establishment Registration shall be valid for three (3) years and for its renewal, it shall comply with the registration requirements set out in this section, as appropriate. The application for renewal of Establishment Registration for businesses which process, place on the market, import and/or exports, animal/plant origin products and by-products, must be submitted to SENASA at least with (60) calendar days prior to the expiry date of the current registration.

6.10 OBTAINING PERMITS FROM THE NATIONAL ANIMAL HEALTH SERVICE (SENASA) 6.10.1 DOCUMENTS TO BE SUBMITTED FOR THE REGISTRATION OF ESTABLISHMENT OF IMPORTERS OF PRODUCTS AND BY-PRODUCTS OF ANIMAL AND/OR PLANT ORIGIN Who performs the procedure? Registration form (codified) addressed to the Director-General of the National Service of Health and Food Safety by: • The Legal Representative, who must certify his representation by means of Public Deed or, failing that, Power of Attorney; or; • The Legal Proxy, by means of duly authenticated Power of Attorney. b. Payment receipt for fee for Establishment Registration service as indicated in the Regulations of Fees for Services provided by the National Service of Health and Food Safety, SENASA and its validity. c. Payment receipt of registration inspection fee, as established in the Regulation of Fees for Services provided by the National Service of Health and Food Safety and its validity. d. Payment receipt of the transfer fee and shipment of water samples, as established in the Regulation of Fees for Services provided by the National Service of Health and Food Safety and its validity.
