Company´s Articles of Incorporation and appointment of representatives. i. Current Operation Permit j. Name of the technical adviser responsible for import and export with valid license. k. Summary description of the facilities available for the safe storage, packaging, packing and handling of the product and waste l. Contingency plan. m. Program of epidemiological surveillance of operators’ health. n. Quality control of packages. *All documents for which copies are submitted must be duly authenticated or cross-checked against the original in the General Secretariat of the Ministry of Agriculture and Livestock. VALIDITY: The Register of Establishment shall be valid for 2 years and for its renewal, request shall comply with the registration requirements set out in this section, as appropriate. Any request for renewal must be made 30 days before expiration. 6.10.6 APPLICATION FOR REGISTRATION OF EXPORTER OF FERTILIZERS AND PESTICIDES The application is regulated under the Phytozoosanitary Law, Decree No 157-94, art: 14 and 15 - Regulations on use and control of pesticides, art: 6, 7, 8, 9, 51 subsection “b”, 194, 195, 196 y 197. Current Regulation Agreement No. 005-2019 of SENASA service fees art: 3 numeral "4”. REQUIREMENTS: a. Application for registration addressed to the Director General of the National Service of Health and Food Safety submitted by the legal representative of the establishment which conferred his power.
