Termination Form The Termination Form was initiated throughout CCMH counseling centers beginning July 2015. Clinicians are asked to complete the Termination Form following the end of individual treatment with a client. This graph describes the frequency of each termination reason for students that received individual treatment during the 2016-2017 academic year (N= 19,632).
