Termination Form The Termination Form was initiated throughout CCMH counseling centers beginning July 2015. Clinicians are asked to complete the Termination Form following the end of individual treatment with a client. Clinicians can “select all that apply” from a checklist of 17 reasons why individual therapy may have ended for a given client. This graph describes the frequency of each termination reason for students that received individual treatment during the 2016-2017 academic year (N= 19,632).

Departure of provider Graduation of client Referred out to a higher level/specialized care Other Referred out for continuation of services Treatment goals completed Client/provider mutual agreement End of academic term (semester/quarter) Client did not return (e.g., no-show, cancellation, no response, etc.) Reason (check all that apply)

Voluntary withdrawal from institution Service limit(s) were reached in center

Transferred to another provider within center Termination against provider recommendation Transferred to different treatment modality within center

Financial reasons Involuntary withdrawal from institution Ineligible for services in center
