Eligibility You and your dependents are eligible for benefits on the first of the month following your date of hire. Eligible dependents are your spouse and children up to the age of 26. Elections made now will remain until the next open enrollment unless you or your family members experience a qualifying event. If you experience at qualifying event, you must contact HR within 30 days. “Free” Benefits Sartorius pays the entire cost of some of your benefits and you are automatically enrolled upon meeting eligibility. NEW 2019 Wellness Premium Credit For completing certain wellness activities, employees will receive a wellness premium credit of up to $160 for an individual, or $320 for all other coverage types for 2019. Employees will have a new wellness activity to complete quarterly.

Health Savings Account (HSA) If you enroll in the Sartorius HSA-Qualified High Deductible Health Plan, you may elect to participate in a Health Savings Account (HSA). The HSA is a tax-advantaged account that allows you to fund qualified healthcare expenses with pre-tax contributions. HSA’s are owned by you and, as a result, are portable. In other words, your ownership of the HSA balance is not affected by a change in employment, there is no use-it-or-lose it provision. If you are eligible for HSA contributions, Sartorius will fund your HSA through pre-tax payroll contributions up to $1,000 per year for those with employee-only coverage and $2,000 per year for those with employee/spouse, employee/child(ren) or full family coverage. Employer contributions will be provided of the first payroll cycle of each quarter. Funds in an HSA may be used to pay qualified medical, pharmacy, dental and vision expenses at any time without federal tax liability. Keep in mind, however, withdrawals for non-qualified expenses are treated very similarly to early withdrawals from an IRA account in that they may provide tax advantages if taken after retirement age, and they incur penalties if taken earlier.

• Up to $3,500 for a single in 2019 • Up to $7,000 for a family in 2019 • If you are age 55 or older, you can make an additional “catch up” contribution of $1,000 • Tax-free growth on account balances • Tax-free withdrawals for eligible expenses

• Unused balances carry forward • You own your HSA; it’s portable • You have three deposit options available • Arrange a regular automatic payroll deduction • Mail a check to be deposited in your account

• Arrange a regular electronic transfer from an account at another financial institution • You will receive a debit MasterCard from your HSA Bank to withdraw funds to pay for eligible healthcare expenses

Using your HSA You may use the money in your HSA to pay for any “qualified healthcare expenses” permitted under federal tax laws for yourself, your spouse or your dependent children. You can pay for expenses of your spouse and dependent children even if they are not covered by your HDHP such as: medical care and services, dental, vision, COBRA continuation, long term care insurance, and Medicare premiums. If you are enrolled in Medicare, you are no longer eligible to contribute to your HSA. It is your responsibility to let Human Resources know that you are enrolled in Medicare.

Flexible Spending Account A flexible spending account allows employees to use the pre-tax money set aside to pay for qualifying out-of- pocket healthcare, dental, vision or hearing expenses. Out-of-pocket expenses are those that are not covered by their existing insurance plans. These expenses include deductibles, coinsurance and co-pays and certain over-the- counter (OTC) expenses. Healthcare Spending Limit: $2,700 per year maximum Note: If you enroll in the High Deductible Health Plan you can elect our limited purpose FSA for dental and vision expenses only. Dependent Care Account A dependent day care spending account allows employees to use the pre-tax money set aside to pay for child or adult day care expenses. These expenses include day care, before-and-after school programs, nursery school or preschool, summer day camp and even adult day care. Dependent Care Spending Limit: $5,000 per year maximum

Disability Insurance Short-term and Long-term Disability rates are based on the employee’s salary.
