Paradoxes in Early Intervention & Rehabilitation Symposium Organizer: Sharon Landesman Ramey (USA) Affiliation : Virginia Tech
A broad integrative assessment of the results from randomized controlled trials (RCTs) and longitudinal studies extending from infancy to adulthood reveals a set of paradoxes concerning efficacious early interventions for babies born with a wide range of risk conditions and/or CNS injuries or medical conditions. This symposium will review these paradoxes and how they support adopting an explicit total-child conceptual framework to inform both treatment decisions and the design of future scientific studies of innovative and combined interventions. IMPACT2 (Interdisciplinary Monitoring, Planning, and Caring for the Total Child - Together Developmental Framework) emphasizes the imperative for strong and flexible partnerships, including the family, to realize primary and secondary prevention and to optimize functional outcomes across the life course. Specific findings about high-impact treatment strategies for: (1) low birthweight and premature infants; (2) infants with prenatal or perinatal stroke and children with diagnosed CNS conditions; and (3) children born into families with extremely low economic, educational, and/or social resources are remarkably consistent. A common total-child framework can guide identification of the types of child-initiated transactions with others (adults, peers, treatment specialists) and the physical environments and activities that provide immediate and practically useful feedback for the child (most notably, response-contingent reinforcement and continuous shaping).
